研究目的：

本研究旨在系统探讨高原环境对骨量减少和慢性骨关节炎的发生与进展的独特影响，构建精准管理及防治策略的科学依据。通过建立长期随访的高原人群队列数据库，解析疾病的风险因素、共病模式及其病理机制，最终形成针对性强、可实施的个体化干预方案。具体目标包括： （1）构建久居高原脱习服人群骨量减少和慢性骨关节炎的队列研究数据库，开展多维度随访与数据采集。 （2）评估疾病的关键影响因素，开发可解释性的风险评估模型并揭示两种高原运动系统慢性疾病的共病模式。 （3）探索骨量减少和膝关节骨关节炎的表观遗传及代谢调控机制，为疾病预防、诊断及治疗提供生物学依据。  

Objectives of Study：

This study aims to systematically investigate the unique impact of the plateau environment on the occurrence and progression of osteopenia and chronic osteoarthritis, thereby establishing a scientific basis for precise management and prevention strategies. By constructing a long-term follow-up cohort database of plateau residents, we will analyze the risk factors, comorbidity patterns, and pathological mechanisms of these diseases, ultimately formulating targeted and implementable individualized intervention regimens. Specific objectives include: (1) Establishing a cohort study database for osteopenia and chronic osteoarthritis in long-term plateau residents undergoing de-acclimatization, and conducting multidimensional follow-up and data collection. (2) Evaluating key influencing factors of the diseases, developing interpretable risk assessment models, and revealing the comorbidity patterns between these two chronic musculoskeletal diseases prevalent on the plateau. (3) Exploring the epigenetic and metabolic regulatory mechanisms underlying osteopenia and knee osteoarthritis to provide a biological basis for disease prevention, diagnosis, and treatment.

纳入标准：

Inclusion criteria

排除标准：

1.暴露组（久居高原脱习服人群）；
1）最近1年内有6个月及以上的高原旅居史人群；
2）膝骨关节炎K-L分级Ⅲ级及以上患者；
3）非椎体骨折患者；
4）继发性骨质疏松症患者，包括：I 型糖尿病（胰岛素依赖型）、成年成骨不全症、未治疗的长期甲状腺机能亢进、性腺机能减退或过早绝经（小于45岁）、慢性营养不良或吸收不良以及慢性肝病）；
5）风湿性关节炎患者；
6）目前正口服肾上腺皮质激素或曾经口服过肾上腺皮质激素超过三个月以上患者；
7）恶性肿瘤患者；
8）其它研究者认为不适宜纳入的情况。
2.（2）非暴露组（久居平原人群）；
1）最近5年内有6个月及以上的高原旅居史人群；
2）膝骨关节炎K-L分级Ⅲ级及以上患者；
3）非椎体骨折患者；
4）继发性骨质疏松症患者，包括：I 型糖尿病（胰岛素依赖型）、成年成骨不全症、未治疗的长期甲状腺机能亢进、性腺机能减退或过早绝经（小于45岁）、慢性营养不良或吸收不良以及慢性肝病）；

Exclusion criteria：

1. Exposed group (de-acclimatized Han population returned to low-altitude areas after long-term high-altitude residence);
1) Individuals with a history of plateau residence for 6 months or more within the past year;
2) Patients with knee osteoarthritis of K-L grade III and above;
3) Patients with nonvertebral fractures;
4) Patients with secondary osteoporosis, including: Type 1 diabetes (insulin-dependent), adult-onset osteogenesis imperfecta, untreated long-standing hyperthyroidism, hypogonadism or premature menopause (less than 45 years old), chronic malnutrition or malabsorption, and chronic liver disease);
5) Patients with rheumatoid arthritis;
6) Patients who are currently taking oral adrenocortical hormones or have taken oral adrenocortical hormones for more than three months;
7) Malignant tumor patients;
8) Other situations judged by researchers as ineligible for inclusion.
2.(2) Non-exposed group (long-term residents of plains);

2) Patients with knee osteoarthritis of K-L grade III and above;
3) Patients with nonvertebral fractures;
4) Patients with secondary osteoporosis, including: Type 1 diabetes (insulin-dependent), adult-onset osteogenesis imperfecta, untreated long-standing hyperthyroidism, hypogonadism or premature menopause (less than 45 years old), chronic malnutrition or malabsorption, and chronic liver disease);
5) Patients with rheumatoid arthritis;
6) Patients who are currently taking oral adrenocortical hormones or have taken oral adrenocortical hormones for more than three months;
