研究目的:

“组织病理诊断”作为宫颈癌诊断的金标准,受限于阴道镜医师主观判断以及宫颈疾病的跳跃性,故易漏诊及过度诊疗。从宫颈癌的发生规律可知,病变由基底层开始逐步向上蔓延,HISL病变是干预的指征,TCT诊断具有特异性高特点是因为其采样过程中覆盖面广,形态学检查贴合金标准。我们假设宫颈表面脱落细胞能够产生特异性的代谢物,那么联合HPV及宫颈表面细胞代谢物或上游基因检测能够提高目前宫颈癌筛查的敏感性及特异性,由此可避免组织活检。本项目利用宫颈脱落细胞进行代谢组学分析,动态构建单纯HPV感染、LSIL、HSIL及宫颈鳞癌代谢指纹图谱,并选取代谢物进行诊断建模,通过生信分析探索疾病变化过程中发生的致癌机制,为临床诊断及治疗提供新思路。  

Objectives of Study:

"Histopathological diagnosis" is considered the gold standard for cervical cancer diagnosis. However, it is limited by the subjective judgment of colposcopy physicians and the skip nature of cervical diseases, leading to potential missed diagnoses and over-treatment. From the progression patterns of cervical cancer, it is known that the lesion starts from the basal layer and gradually spreads upwards, with HSIL (High-Grade Squamous Intraepithelial Lesion) being the indication for intervention. The high specificity of TCT (ThinPrep Cytologic Test) diagnosis is attributed to its wide sampling coverage and morphological examination that aligns with the gold standard. We hypothesize that exfoliated cervical cells can produce specific metabolites. Therefore, the combination of HPV and cervical surface cell metabolites or upstream gene detection could enhance the sensitivity and specificity of current cervical cancer screening, potentially avoiding the need for tissue biopsies. This project utilizes metabolomic analysis of exfoliated cervical cells to dynamically construct metabolic fingerprint profiles for pure HPV infection, LSIL (Low-Grade Squamous Intraepithelial Lesion), HSIL, and cervical squamous cell carcinoma. Metabolites are selected for diagnostic modeling, and bioinformatics analysis is conducted to explore the carcinogenic mechanisms during the disease progression, offering new insights for clinical diagnosis and treatment.

排除标准:

1. 妊娠、哺乳期; 2. 有宫颈切除史,宫颈瘤变史,盆腔放射治疗史; 3. 合并生殖系统急性炎症; 4. 合并严重心、肝、肾功能不全; 5. 合并糖尿病、免疫系统疾病; 6. 存在凝血功能异常。

Exclusion criteria:

1. Pregnancy or breastfeeding. 2. History of cervical excision, cervical neoplasia, or pelvic radiotherapy. 3. Concurrent acute inflammation of the reproductive system. 4. Concurrent severe impairment of heart, liver, or kidney function. 5. Concurrent diabetes or immune system diseases. 6. Coagulation function abnormalities.

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture:

Data will be collected and managed in two parts: 1. Case Record Form (CRF): A standardized CRF covering baseline characteristics, interventions, follow-up outcomes and adverse events is designed by the data manager according to the protocol. 2. Electronic Data Capture (EDC): An internet-based EDC system (ResMan Public Management Platform, http://www.medresman.org) is employed for real-time data entry, automatic validation and secure storage. The electronic CRF (eCRF) built in the EDC is completed by trained investigators or CRCs through individual accounts. The data manager prepares the eCRF completion guidelines, performs data queries and locks the database after verification to ensure integrity, accuracy and traceability
