研究目的：

上颌磨牙腭根是三根之中最大者，因此腭根拔牙窝方向可以作为即刻种植时植入方向的参考，直接将种植体植入上颌磨牙腭根的拔牙窝，从而简化上颌磨牙即刻种植手术，缩短治疗周期，降低费用，减小手术创伤，避免进行上颌窦提升手术可能发生的一些重大风险及并发症，但相关的疗效仍缺乏充足的循证医学证据支持。本研究拟通过比较上颌磨牙腭根即刻种植与常规延期种植的临床效果，以期为上颌磨牙腭根即刻种植提供医学证据支持。  

Objectives of Study：

The palatal root of the maxillary molar is the largest among the three roots, hence the direction of the palatal root extraction socket can serve as a reference for the implantation direction during immediate implantation. By directly implanting the implant into the extraction socket of the palatal root of the maxillary molar, the procedure of immediate implantation in maxillary molars can be simplified, the treatment period can be shortened, costs can be reduced, surgical trauma can be minimized, and some significant risks and complications associated with sinus augmentation surgery can be avoided. However, there is still a lack of sufficient evidence-based medical evidence to support the efficacy. This study aims to provide medical evidence for immediate implantation in the palatal root of maxillary molars by comparing the clinical outcomes of immediate implantation with conventional delayed implantation.

纳入标准：

Inclusion criteria

排除标准：

1. 有系统性疾病可能影响术后愈合期和/或骨结合者，如未控制的糖尿病、高血压等； 2. 系统或局部的骨性疾病，如骨结核，骨炎，骨肿瘤等； 3. 长期服用可能影响术后愈合或骨结合的特殊药物者； 4. 患有口腔外科手术禁忌症患者； 5. 近3年内进行放射治疗或化学治疗的患者； 6. 尚未控制的牙周炎； 7. 酒精依赖、吸毒、烟草或其他药物成瘾者； 8. 研究者认为其他不适合试验的情况。

Exclusion criteria：

1. Individuals with systemic diseases that may affect postoperative healing and/or osseointegration, such as uncontrolled diabetes or hypertension; 2. Individuals with systemic or localized bone diseases, such as osteo-tuberculosis, osteitis, or bone tumors; 3. Long-term use of medications known to interfere with postoperative healing or osseointegration; 4. Individuals with contraindications to oral surgical procedures; 5. Individuals who have undergone radiotherapy or chemotherapy within the past three years; 6. Uncontrolled periodontitis; 7. Individuals with alcohol dependence, drug abuse, tobacco addiction, or other substance addictions; 8. Any other condition deemed unsuitable for the study by the investigators.
