本研究旨在开发一个基于人工智能算法的脓毒症早期预警专家系统, 并结合 SHAP方法对预测结果进行解释。通过整合患者的电子病历和实时生理数据,利用机器学习模型实现对脓毒症的早期识别与预警。同时,通过 SHAP 提供的可解释性分析,使得医护人员能够理解和信任模 型的决策过程,从而提高临床应用的可操作性和有效性。本研究的最终目标是推 动该系统的临床应用转化,为脓毒症的早期诊断和干预提供新的工具,降低病人的死亡率并优化 ICU 治疗方案。  

Objectives of Study:

This study aims to develop an artificial intelligence algorithm-based expert system for early sepsis warning, combined with the SHAP method to interpret prediction results. By integrating patients' electronic medical records and real-time physiological data, machine learning models are employed to achieve early identification and warning of sepsis. Simultaneously, the interpretability analysis provided by SHAP enhances healthcare providers' understanding and trust in the model's decision-making process, thereby improving the practicality and effectiveness of clinical applications. The ultimate goal of this research is to advance the system's clinical application and translation, providing novel tools for early sepsis diagnosis and intervention, reducing patient mortality, and optimizing ICU treatment protocols.

纳入标准:

Inclusion criteria

排除标准:

1.年龄小于18岁;
2.妊娠、原有慢性器官功能衰竭或存在不可逆性器官衰竭或肿瘤晚期患者;
3.多次因脓毒症入院,仅分析首次入院;
4.在外院诊断为脓毒症并且入住外院重症医学科超过24小时的患者;
5.进入重症医学科后超过24小时被诊断为脓毒症患者;
6.入住重症医学科时间超过28天死亡的患者;

Exclusion criteria:

1.Patients under the age of 18;
2.Pregnant patients, those with pre-existing chronic organ failure, or those with irreversible organ failure or advanced tumors;
3.Multiple hospitalizations due to sepsis were conducted, and only the first admission was analyzed;
4.Patients diagnosed with sepsis in other hospitals and admitted to the intensive care unit of other hospitals for more than 24 hours;
5.More than 24 hours after entering the intensive care unit, the patient was diagnosed with sepsis;
6.Patients who died after staying in the intensive care unit for more than 28 days;
