本课题旨在探究基于近红外光谱技术的组织氧饱和度(StO₂)微循环监测在脓毒症(Sepsis)患者病情严重度评估和预后预测中的应用价值。通过检测患者多个部位的局部StO₂值,运用体表面积加权法计算加权全身StO₂值,并结合外周灌注指数(PPI)、毛细血管再充盈时间(CRT)、血乳酸水平、6小时乳酸清除率及动静脉二氧化碳分压差 [Pa-v]CO₂等微循环指标,分析其与APACHEⅡ评分、SOFA评分、机械通气时长、ICU住院日等病情严重度指标的相关性,力求证实加权StO2与脓毒症严重度的密切关联。鉴于单一指标预测预后的局限性,本研究综合微循环数据与其它临床参数,运用多种机器学习算法构建预测模型,经模型性能评估和交叉验证,筛选出最优的28天死亡风险预测模型。本研究期望寻求一种无创、实时、经济且简便、各医疗机构均可开展的脓毒症微循环监测方法,以精准反映脓毒症病情,并通过整合临床参数实现对预后的可靠预测,从而支持临床决策,优化治疗方案,降低医疗成本,减轻患者负担。同时,为下一步以微循环灌注为目标的脓毒症及脓毒性休克管理策略提供前期研究基础。 |

Objectives of Study: |
This project aims to explore the application value of tissue oxygen saturation (StO2) microcirculation monitoring based on near-infrared spectroscopy technology in the assessment of the severity of the disease and prognosis prediction of Sepsis patients. By detecting the local StO2 values of multiple parts of the patients, the weighted systemic StO2 value was calculated using the body surface area weighting method, and combined with microcirculation indicators such as peripheral perfusion index (PPI), capillary refillation time (CRT), blood lactate level, 6-hour lactate clearance rate, and arteriovenous carbon dioxide partial pressure difference [PAV]CO2, To analyze its correlation with severity indicators such as APACHEⅡ score, SOFA score, duration of mechanical ventilation, and length of stay in the ICU, and strive to confirm the close association between weighted StO2 and the severity of sepsis. Given the limitations of a single indicator in predicting prognosis, this study integrated microcirculation data with other clinical parameters and employed multiple machine learning algorithms to construct a prediction model. Through model performance evaluation and cross-validation, the optimal 28-day mortality risk prediction model was screened out. This study aims to seek a non-invasive, real-time, economical and simple microcirculation monitoring method for sepsis that can be carried out by all medical institutions, in order to accurately reflect the condition of sepsis and achieve reliable prognosis prediction by integrating clinical parameters, thereby supporting clinical decision-making, optimizing treatment plans, reducing medical costs and alleviating the burden on patients. Meanwhile, it provides a preliminary research basis for the next step of sepsis and septic shock management strategies targeting microcirculation perfusion. |

1)孕产妇;2)继发严重心律失常、急性心肌梗塞、肺动脉栓塞;3)有心、肝、肾移植手术史;4)大面积烫伤、烧伤,皮肤破溃、感染,不宜放置组织氧饱和度监测探头者。 |

Exclusion criteria: |
1)Pregnant and postpartum women; 2) Secondary severe arrhythmia, acute myocardial infarction, and pulmonary embolism; 3) Have a history of heart, liver or kidney transplantation surgery; 4) Those with extensive burns, scalds, skin ulcers or infections, and who are not suitable for placing tissue oxygen saturation monitoring probes. |
