研究目的:

本研究通过视觉模拟评分量表和NPDS障碍指数以及相应得纳入排除标准共纳入CNP受试者152名。评估 VR 训练干预与 MCE 干预对CNP患者颈部肌肉激活时序、激活强度及颈椎本体感觉的影响。受试者被随机分为四组:VR 训练组、MCE 训练组、VR 联合 MCE 训练组,以及疼痛神经生理教育(Pain neurophysiology education,PNE)组。所有受试者在干预前后均使用sEMG和 Vicon 动作捕捉系统进行评估。研究的主要目标是比较上述干预策略颈部肌肉激活与头部本体感觉功能的影响,包括单独 VR 训练、单独 MCE 训练、两者联合应用以及 PNE 的效果。该设计将帮助我们探讨 VR 与 MCE 训练在缓解慢性颈痛方面的潜在益处,并判断两者之间是否存在协同效应。所有数据将进行统计学分析,以确定各组间差异是否具有统计学意义,从而进一步揭示不同干预策略对颈部肌肉功能及头颈部本体感觉的具体影响。  

Objectives of Study:

A total of 152 participants with chronic neck pain (CNP) were enrolled according to the visual analogue scale (VAS), the Neck Pain and Disability Scale (NPDS), and the corresponding inclusion and exclusion criteria. This study evaluates the effects of virtual-reality (VR) training and motor-control exercise (MCE) on neck-muscle activation timing, activation intensity, and cervical proprioception in CNP patients. Participants were randomly allocated to four groups: VR training alone, MCE training alone, combined VR + MCE training, or pain neurophysiology education (PNE). Pre- and post-intervention assessments were conducted with surface electromyography (sEMG) and a Vicon motion-capture system. The primary objective is to compare the effects of these four intervention strategies on neck-muscle activation and head/neck proprioception, examining VR alone, MCE alone, their combination, and PNE. This factorial design will clarify the potential benefits of VR and MCE for alleviating chronic neck pain and determine whether a synergistic effect exists. All data will be analysed statistically to identify significant between-group differences, thereby providing deeper insight into the specific impacts of each intervention on cervical muscle function and proprioception.

Inclusion criteria

排除标准:

①既往接受过颈椎手术(手术可能改变颈椎的生物力学和功能状态); ②患有严重的神经系统疾病(如帕金森病、多发性硬化等),以及严重的内科疾病(如心脏病、未控制的高血压等); ③疼痛主要来源于非颈椎部位(如肩部、上肢神经根病变等),或存在其他可能导致颈部疼痛的疾病(如颈椎结核、颈椎肿瘤等); ④并发肝、肾、造血系统、内分泌系统等严重原发性疾病; ⑤并发皮肤疾病者; ⑥孕妇及哺乳期妇女;精神病受试者; ⑦在研究前1个月内接受过可能影响颈椎功能的治疗,包括药物治疗。

Exclusion criteria:

1 Previous cervical spine surgery (surgery may alter cervical biomechanics and functional status); 2 Severe neurological disorders (e.g., Parkinson’s disease, multiple sclerosis) or serious internal medical conditions (e.g., cardiac disease, uncontrolled hypertension); 3 Pain primarily originating from non-cervical regions (e.g., shoulder, upper-limb radiculopathy) or other disorders that may cause neck pain (e.g., cervical tuberculosis, cervical tumor); 4 Concurrent severe primary diseases of the liver, kidneys, hematopoietic system, endocrine system, or other vital organs; 5 Concurrent skin disorders; 6 Pregnant or lactating women; individuals with psychiatric disorders; 7 Receipt of any treatments that could influence cervical function, including pharmacotherapy, within the past month.
