研究目的:

本次研究试图在明确慢性踝关节不稳(Chronic Ankle Instability, CAI)运动员单腿落地时的中枢、外周神经异常活动的基础上,施加靶向的经颅直流电刺激(Transcranial Direct Current Stimulation,tDCS)、神经肌肉电刺激(Neuromuscular Electrical Stimulation,NMES)、联合电刺激,探讨不同电刺激方案对该群体单腿落地稳定性的改善效果差异,进而明确最佳刺激方案。  

Objectives of Study:

This study aims to investigate the differences in the effects of various electrical stimulation protocols on single-leg landing stability in athletes with Chronic Ankle Instability (CAI), based on an understanding of the abnormal central and peripheral neural activity during single-leg landings. Specifically, targeted Transcranial Direct Current Stimulation (tDCS), Neuromuscular Electrical Stimulation (NMES), and combined electrical stimulation will be applied to explore their impacts on stability improvement in this population. Ultimately, the goal is to identify the most effective stimulation protocol

Inclusion criteria

排除标准:

1. 有下肢手术或骨折史; 2. 在临近开展研究时发生踝关节扭伤; 3. 有严重损害认知功能的病史,如脑震荡、学习障碍等; 4. 有平衡或前庭障碍病史; 5. 踝关节距骨倾斜试验、前抽屉试验阳性者; 6. 正在服用可能影响其认知障碍的药物,如麻醉剂、抗抑郁剂、抗焦虑剂等。

Exclusion criteria:

1. History of lower limb surgery or fracture; 2. Ankle sprain occurring shortly before the study initiation; 3. History of conditions severely impairing cognitive function, such as concussion or learning disabilities; 4. History of balance or vestibular disorders; 5. Positive talus tilt test and anterior drawer test; 6. Currently taking medications that may affect cognitive function, such as anesthetics, antidepressants, or anxiolytics.

数据采集和管理(说明:数据采集和管理由两部分组成,一为病例记录表(Case Record Form, CRF),二为电子采集和管理系统(Electronic Data Capture, EDC),如ResMan即为一种基于互联网的EDC:

数据采集 本研究为干预性试验,采集研究对象下肢落地稳定性相关生物力学指标、电刺激干预相关参数及一般人口学信息。生物力学指标通过三维运动捕捉系统、测力台同步采集,包括落地时关节角度、地面反作用力、落地时间等核心指标;电刺激参数记录刺激频率、强度、干预时长及刺激部位;人口学信息含年龄、性别、身高、体重、运动史等。所有指标均严格按研究方案标准化操作采集,采集前对操作人员统一培训,确保操作一致性;采集过程实时记录,避免遗漏,异常数据当场标注原因。 数据管理 ① 数据录入:采集所得原始数据双人双录至加密 Excel 数据库,录入完成后交叉核对,确保无录入误差;② 数据核查:先通过逻辑核查剔除明显异常值,再结合原始采集记录人工复核关键指标,质疑数据需溯源原始记录澄清;③ 数据存储:原始数据及录入数据采用本地电脑 + 移动硬盘双备份,均设置密码保护;④ 数据溯源:所有数据关联研究对象唯一编号,采集时间、操作人员、仪器编号全程记录,确保全程可追溯;⑤ 数据保密:研究对象信息去标识化处理,仅研究人员可访问数据库,严格保护受试者隐私。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture:

Data Collection This study is an interventional trial. We collected biomechanical indicators related to lower limb landing stability, relevant parameters of electrical stimulation intervention, and general demographic information of the subjects. Biomechanical indicators were synchronously collected using a 3D motion capture system and a force plate, including core indicators such as joint angles, ground reaction forces, and landing time during landing. Electrical stimulation parameters included stimulation frequency, intensity, intervention duration and stimulation site. Demographic information covered age, gender, height, weight and exercise history. All indicators were collected strictly in accordance with standardized procedures in the study protocol. Operators received unified training prior to data collection to ensure consistent operation. Real-time recording was conducted during the collection process to avoid omissions, and the causes of abnormal data were marked on-site immediately. Data Management ① Data entry: The collected raw data were double-entered into an encrypted Excel database by two independent researchers, and cross-checked after entry to ensure no input errors. ② Data verification: Obvious outliers were first excluded via logical verification, then key indicators were manually reviewed in combination with original collection records. The causes of questionable data were clarified by tracing back to the original records. ③ Data storage: Raw data and entered data were dual-backed up on local computers and mobile hard drives, all protected by passwords. ④ Data traceability: All data were linked to the unique identification number of each subject. The collection time, operator and instrument number were recorded throughout the process to ensure full traceability. ⑤ Data confidentiality: Subject information was de-identified. Only researchers had access to the database to strictly protect the privacy of subjects.
