研究目的：

对老年骨质疏松性再骨折预防方案进行验证，通过随机对照试验验证该方案在改善患者不良临床结局方面的效果，同时探讨老年人对分群干预过程及效果的接受程度及预防方案的临床可行性。  

Objectives of Study：

The prevention program for osteoporotic subsequent fracture in the elderly will be verified. The effect of this program in improving the adverse clinical outcomes of patients will be verified through randomized controlled trials. At the same time, the acceptance degree of the stratified intervention process and effect among the elderly and the clinical feasibility of the prevention program will be explored.

纳入标准：

Inclusion criteria

排除标准：

（1）病情危重或病情随时会变化者；（2） 假体周围骨折、陈旧性骨折或病理性骨折患者；（3）认知功能障碍者。

Exclusion criteria：

(1) Those with critical illness or conditions that may change at any time; (2) Patients with periprosthetic fractures, old fractures, or pathological fractures; (3) Individuals with cognitive impairment.

干预措施：

干预组采用分群干预模式进行干预。干预流程整体按照分群评估结果匹配适合的干预模块，采用阶段性干预实施。院内干预阶段由干预小组开展线下干预。院外干预阶段，采用家庭访视、电话随访、门诊复诊随访和即时通讯技术进行干预。 再骨折预防分群干预包括：再骨折标准化预防、动机干预、计划干预、行动干预。再骨折预防标准化干预包括：入院护理（入院宣教、体位护理、功能锻炼、饮食指导、辅助检查、入院评估等）、术前护理（术前准备、术前教育）、术后护理（手术交接、病情观察、术后宣教）、并发症预防及护理（关节脱位、静脉血栓栓塞、肺部感染、血管和神经损伤、谵妄、贫血、关节感染）、健康教育及出院指导等方面护理内容。所有干预组的参与者都执行标准化预防方案。动机干预、计划干预、行动干预需根据再骨折预防行为分群评估工具对患者进行分群，以匹配合适的干预模块。

The intervention group was intervened using the clustering intervention model. The overall intervention process is matched with suitable intervention modules based on the results of cluster assessment, and phased intervention is implemented. During the in-hospital intervention stage, the intervention team conducts offline intervention. During the out-of-hospital intervention stage, home visits, telephone follow-ups, outpatient follow-up visits and instant messaging technology were adopted for intervention. The stratified intervention for refracture prevention includes: standardized prevention of refractures, motivational intervention, planned intervention, and action intervention. Standardized interventions for refracture prevention include: Admission care (admission education, position care, functional exercise, dietary guidance, auxiliary examination, admission assessment, etc.), preoperative care (preoperative preparation, preoperative education), postoperative care (surgical handover, condition observation, postoperative education), complication prevention and care (joint dislocation, venous thromboembolism, pulmonary infection, vascular and nerve injury, delirium, anemia, joint infection) Nursing contents such as health education and discharge guidance. All participants in the intervention groups implemented a standardized prevention program. Motivational intervention, planned intervention and action intervention need to group patients according to the refracture prevention behavior clustering assessment tool to match the appropriate intervention module.

对照组采用常规护理，由研究团队成员进行口头健康宣教，并为对照组对象提供医院统一为患者制定的教育手册、康复指导。每两周电话随访患者一次。
