研究目的：

主要目的目前临床上暂无系统的SCD危险评估体系。研究人员在建立更直接反映心律失常底物的生物标志物以及开发侵入性和无创技术以识别有SCD风险的患者方面，已经付出了相当大的努力。高频QRS波在SCD的预防中较为显著。本项目旨在研究基于高频QRS心脏全息检测技术对于SCD患者危险分层，为SCD的高危患者提供更为科学、更为完善、更具有个性化的一级、二级预防。次要目的通过监测不良事件发生率(包括肌痛、肝肾功能异常、磷酸激酶升高、新发糖尿病等)，评估不同危险分层患者的安全性差异，为临床风险-获益评估提供依据。

Objectives of Study：

Primary purpose At present, there is no systematic SCD risk assessment system in clinical practice. Researchers have put considerable effort into establishing biomarkers that more directly reflect arrhythmia substrates, as well as developing invasive and noninvasive techniques to identify patients at risk for SCD. High-frequency QRS complex is more significant in the prevention of SCD. The purpose of this project is to study the risk stratification of SCD patients based on high-frequency QRS cardiac holographic detection technology, and to provide more scientific, complete and personalized primary and secondary prevention for high-risk patients with SCD. Secondary purpose By monitoring the incidence of adverse events (including myalgia, abnormal liver and kidney function, elevated phosphokinase, new-onset diabetes mellitus, etc.), the safety differences of patients with different risk strata were evaluated, and the clinical risk-benefit assessment was provided.

纳入标准：

入选标准
（一）阶段一：健康体检组
1.	年龄18-75周岁；
2.	无冠心病、心肌病等器质性心脏病史；
3.	签署知情同意书。
（二）阶段一及阶段二：室速、室颤或心脏猝死高危人群组
1.	年龄大于18周岁。
2.	有记录的室速、室颤或心脏猝死高危人群符合以下条件之一：
有记录（心电图、Holter、床边及便携式心电监护、植入式器械程控资料，包括心电事件记录仪、ICD等）的持续性室速、室颤患者或心脏猝死高危人群。
3.	非可逆性原因导致的室颤或血流动力学不稳定的持续性室性心动过速，引起的心脏猝死高危人群（包括ICD正确电治疗含ATP及电复律患者）。
4.	合并自发持续室性心动过速曾确诊以下器质性心脏疾病患者（包括ICD正确电治疗含ATP及电复律患者）。
1)	心肌梗死40d以上，LVEF≤0.35，心功能II或III级患者；或心肌梗死40d以上，LVEF≤0.30，且心功能I级患者;
2)	非缺血性心肌病，心功能II或III级，LVEF≤0.35的患者;
3)	遗传性心律失常疾病：一组心脏编码离子通道和其调控蛋白基因突变所致的，以心律失常、晕厥和猝死为主要临床表现的遗传性疾病，包括长QT综合征、短QT综合征和Brugada综合征等。
4)	长QT综合征：体表心电图提示QTc大于440ms,伴有快速性室性心律失常、晕厥和猝死临床表现，无器质性心脏病，除外电解质紊乱、药物等因素所致的获得性QT间期延长。
5)	短QT综合征：体表心电图提示QTc小于300ms，伴有快速性室性心律失常、晕厥和猝死临床表现，无器质性心脏病，除外电解质紊乱、交感兴奋等因素所致的获得性QT间期缩短。
6)	Brugada综合征：体表心电图提示V1-V3导联ST段呈下斜型或马鞍形抬高，伴有右束支传导阻滞、快速性室性心律失常、晕厥及猝死临床表现，无器质性心脏病，除外其他因素所致的ST-T改变。
7)	儿茶酚胺敏感性室性心动过速：无心脏结构异常，QTc正常的健康个体在运动平板试验或静滴异丙肾上腺素时出现典型的双向性、多形性室性心动过速。
8)	肥厚型心肌病：经检查（含超声心动图、左心室造影、心脏核磁或心脏CT等）证实非对称性室间隔肥厚＞15mm，或对称性肥厚室间隔厚度／左室后壁厚度<1.3，左心室舒张期顺应性下降，伴或不伴有左心室内或流出道梗阻，需除外高血压、主动脉瓣狭窄等疾病所致心脏改变。
注：以上心律失常必须是有明确记录的，含病程记录、护理记录、心电图、Holter、床边心电监护、遥测心电监护、便携式心电记录仪、植入式器械程控资料，包括心电事件记录仪、起搏器、ICD等。

Inclusion criteria

Inclusion Criteria
(1) Stage 1: Health examination team
1. Age 18-75 years old;
2. No history of structural heart disease such as coronary heart disease, cardiomyopathy;
3. Signed informed consent.
(2) Stage 1 and Stage 2: high-risk groups of ventricular tachycardia, ventricular fibrillation or sudden cardiac death
1. Be over 18 years old.
2. Documented people at high risk of ventricular tachycardia, ventricular fibrillation, or sudden cardiac death meet one of the following criteria:
Patients with sustained ventricular tachycardia, ventricular fibrillation, or people at high risk of sudden cardiac death who have recorded (ECG, Holter, bedside and portable ECG monitoring, program-controlled data of implantable devices, including ECG event recorders, ICDs, etc.).
3. People at high risk of sudden cardiac death caused by ventricular fibrillation or hemodynamically unstable sustained ventricular tachycardia caused by non-reversible causes (including patients with ATP and electrical cardioversion with ICD correct electrical therapy).
4. Patients with spontaneous sustained ventricular tachycardia who have been diagnosed with the following organic heart diseases (including patients with ATP and electrical cardioversion with ICD correct electrical therapy).
1) Patients with myocardial infarction for more than 40 days, LVEF <= 0.35, cardiac function grade II or III, or myocardial infarction for more than 40 days, LVEF <= 0.30, and cardiac function grade I;
2) Patients with non-ischemic cardiomyopathy, cardiac function grade II or III, LVEF <=0.35;
3) Hereditary arrhythmias: a group of hereditary diseases caused by mutations in genes encoding ion channels and their regulatory proteins, with arrhythmia, syncope and sudden death as the main clinical manifestations, including long QT syndrome, short QT syndrome and Brugada syndrome.
4) Long QT syndrome: the body surface electrocardiogram showed that QTc was greater than 440ms, accompanied by tachyventricular arrhythmia, syncope and sudden death, no organic heart disease, except for electrolyte imbalance, drugs and other factors caused by acquired QT interval prolongation.
5) Short QT syndrome: the body surface ECG shows that QTc is less than 300ms, accompanied by tachyventricular arrhythmia, syncope and sudden death, no organic heart disease, except for electrolyte disorders, sympathetic excitement and other factors caused by acquired QT interval shortening.
6) Brugada syndrome: the surface ECG showed that the ST segment of V1-V3 was downward inclined or saddle-shaped, accompanied by right bundle branch block, tachyventricular arrhythmia, syncope and sudden death, no structural heart disease, except for ST-T changes caused by other factors.
7) Catecholamine-sensitive ventricular tachycardia: Healthy individuals with no structural cardiac abnormalities and normal QTc present with typical bidirectional, polymorphic ventricular tachycardia on exercise treadmill test or intravenous isoproterenol.
8) Hypertrophic cardiomyopathy: asymmetric ventricular septal hypertrophy >15mm, or symmetrical hypertrophy of the ventricular septum/posterior wall thickness of the left ventricular wall, confirmed by examination (including echocardiography, left ventricular angiography, cardiac MRI or cardiac CT, etc.). <1.3, decreased diastolic compliance of the left ventricle, with or without left ventricular or outflow tract obstruction, cardiac changes caused by hypertension, aortic stenosis and other diseases should be excluded.
Note: The above arrhythmias must be clearly recorded, including disease course records, nursing records, ECG, Holter, bedside ECG monitoring, telemetry ECG monitoring, portable ECG recorder, implantable device program control data, including ECG event recorder, pacemaker, ICD, etc.

排除标准：

排除标准 1. 怀孕或准备怀孕。 2. 不能或不愿遵守研究方案并完成随访。 3. 有未控制的甲状腺功能亢进症和甲状腺功能减退症、严重感染、严重肝肾功能不全（ALT≧3倍，或／和任何公式计算的eGFR≦30mL/min）、恶性肿瘤等。 4. 经心电图或心电生理检查确诊的各种特发性室性心动过速，包括左后分支、左前分支，右室流出道、左室流出道等特殊部位的特发性室性心动过速等。 5. 各种结构性心脏疾病，包括各种先天性心脏病，风湿性或老年性心脏瓣膜病。 6. 急性或亚急性感染性心内膜炎，急性病毒性心肌炎。 7. 单纯右心功能不全所致的肺动脉高压，经UCG或右心导管检查PASP≧40mmHg。 8. 严重的电解质紊乱、酸碱失衡。 9. 各种严重缓慢心律失常所致心脏骤停。 10. 对胶粘剂或水凝胶类产品有己知的过敏反应。 11. 近期有外科手术损伤，损伤部位为可能黏贴电极的部位等无法完成过电极黏贴及无法完成心电记录。 12. 正在参加其他临床试验，并且可能影响本研究的数据收集。 13. 研究者认为有不适合入组的其他情况。

Exclusion criteria：

Exclusion Criteria: 1. Pregnant or trying to become pregnant. 2. Unable or unwilling to comply with the study protocol and complete follow-up. 3. Have uncontrolled hyperthyroidism and hypothyroidism, severe infection, severe hepatic and renal insufficiency (ALT>= 3 times, or/and eGFR calculated by any formula <=30mL/min), malignant tumors, etc. 4. Various idiopathic ventricular tachycardia confirmed by electrocardiogram or electrocardiophysiological examination, including left posterior branch, left anterior branch, right ventricular outflow tract, left ventricular outflow tract and other special parts. 5. Various structural heart diseases, including various congenital heart diseases, rheumatic or senile valvular heart disease. 6. Acute or subacute infective endocarditis, acute viral myocarditis. 7. For pulmonary hypertension caused by simple right heart insufficiency, PASP >=40mmHg by UCG or right heart catheterization. 8. Severe electrolyte imbalance, acid-base imbalance. 9. Cardiac arrest caused by various severe bradyarrhythmias. 10. Knowingly allergic reactions to adhesives or hydrogel-based products. 11. Have a recent surgical injury, and the injury site is a part where electrodes may be attached, etc., and electrode bonding and electrocardiogram recording cannot be completed. 12. Is participating in other clinical trials and may affect the data collection of this study. 13. Other conditions that are considered by the investigator to be inappropriate for enrollment.
