分析皮肤保护膜联合造口袋与负压引流对比氧化锌软膏在重症急性胰腺炎患者失禁性皮炎预防中的临床效果及成本效果。  

Objectives of Study：

To analyze the clinical and cost-effectiveness of zinc oxide ointment in the prevention of incontinent dermatitis in patients with severe acute pancreatitis.

纳入标准：

Inclusion criteria

排除标准：

患者入科时会阴部或肛周存在任一种皮肤损伤，如IAD、压力性损伤、皮肤撕裂伤等；有影响IAD观察的皮肤病者；中途退出研究。

Exclusion criteria：

Patients presenting with any perineal or perianal skin lesions (such as incontinence-associated dermatitis, pressure injuries, skin tears, etc.) upon admission; patients with dermatological conditions that may interfere with the assessment of incontinence-associated dermatitis; or those who withdraw from the study prematurely.

由独立统计师在研究启动前使用SAS 9.4软件生成随机数字，基于预试验显示的干预效果差异设置分配比例，通过区组随机法（区块长度=6）确保组间周期性平衡。

Random numbers were generated by an independent statistician before study initiation using SAS 9.4 software, setting allocation proportions based on differences in intervention effects as shown in the pretrial trial, and the effect size of the pretrial trial, the periodic balance between groups is ensured by block randomization (block length = 6) .

盲法：

由于干预措施的特性（试验组使用可见的造口袋装置），本研究采用评估者盲法设计。实施干预的护理人员知晓分组情况以确保操作准确性，但负责IAD诊断和严重程度评定的2名伤口造口师不知晓患者分组情况，数据统计人员同样保持盲态。

Blinding：

Because of the nature of the intervention (the trial group used a visible stoma bag device) , this study was designed in a blinded assessor fashion. The nursing staff who implemented the intervention were aware of the grouping to ensure procedural accuracy, but the 2 wound ostiologists who were responsible for IAD diagnosis and severity assessment were unaware of the patient grouping, and the 2 wound ostiologists who performed the intervention were unaware of the patient grouping to ensure procedural accuracy, statisticians, too, remain blind.
