研究目的：

超声乳化白内障摘除术已成为白内障手术的金标准，其切口小、组织损伤轻、手术源性散光低、手术安全稳定和术后视力恢复快等已经快速普及，但术中核处理技术仍然是关键，原位囊袋内超声乳化技术，劈核、乳化均在囊袋内完成也是核处理的金标准。由于受囊袋和前囊撕囊口大小的限制，而且后囊膜较前囊膜薄，最薄处后囊膜后极部约 2~3 μ m，原位囊袋内超声乳化技术超乳头位置较深，稍有不慎，就有造成前囊撕囊口裂开、后囊膜破裂，甚至晶状体核脱入玻璃体腔的风险，白内障核按照硬度递分成 5 级，无论是一名成熟或者初学者两头难的问题仍然存在（即过软核和硬核是术中后囊破裂的高危对象）。

Objectives of Study：

Ultrasound emulsification cataract extraction has become the gold standard of cataract surgery, with its small incision, light tissue damage, low astigmatism of surgical origin, safe and stable surgery, and fast postoperative recovery of vision, etc. However, intraoperative nuclear processing is still the key, and in situ intracapsular ultrasound emulsification, where the nuclear cleavage and emulsification are all done inside the capsule, is also the gold standard of nuclear processing. Due to the limitation of the size of the capsule and anterior capsule tear, and the posterior capsule membrane is thinner than the anterior capsule membrane, with the thinnest point of the posterior capsule membrane being about 2-3 μm at the posterior extremity of the posterior capsule membrane, the in situ intracapsular ultrasound emulsification technique has a deeper position of the papillae, and a slight inadvertent mistake may result in the splitting of the anterior capsule tear, rupture of the posterior capsule membrane, or even risk of the lens nucleus being dislodged into the vitreous cavity, and the cataract nucleus is classified into 5 levels of hardness according to its hardness. The problem of hardness still exists (i.e., overly soft and hard nuclei are at high risk for intraoperative posterior capsule rupture).

排除标准：

1、4 级以上极硬的核，如黑色或深棕色核的白内障； 2、脱位或半脱位白内障； 3、眼球震颤； 4、角膜内皮病变； 5、瞳孔不能充分散大； 6、浅前房； 7、小眼球

Exclusion criteria：

1. Cataracts with extremely hard nuclei of grade 4 or higher, such as black or dark brown nuclei; 2. subluxation or subluxation cataracts; 3. nystagmus; 4. corneal endothelial lesions; 5. failure of the pupil to dilate adequately; 6. shallow anterior chamber; 7. small eyeballs
