研究目的：

通过数据挖掘，构建多重慢病患者（认知障碍为主的老年慢病患者）的健康画像，并与院内认知障碍老年多重慢病患者画像进行对比，对其进行分类分型的早期干预方案构建，为医护人员的诊疗及护理方案提供理论指导。  

Objectives of Study：

Through data mining, we construct health profiles for elderly patients with multiple chronic conditions (primarily cognitive impairment), compare these profiles with in-hospital cognitive-impaired elderly multimorbidity patients, and develop classified early intervention programs. This provides theoretical guidance for clinical diagnosis, treatment, and nursing care strategies.

Inclusion criteria

排除标准：

1.临床资料不完整者；
2.合并精神疾病或晚期癌症、心脑肾严重脏器功能障碍者；
3.自动出院者；

Exclusion criteria：

1.Incomplete clinical data;
2.Suffering from mental illness or terminal cancer and severe organ dysfunction of heart, brain and kidney.
3.Patient discharged against medical advice;

使用病例记录表对多重慢病患者（以认知障碍为主的老年慢病患者）进行相关数据的收集，数据涵盖构建健康画像及早期干预方案所需信息，通过R state和SPSS对数据进行分析和保存管理。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Use the Case Record Form (CRF) to collect relevant data from patients with multiple chronic diseases (elderly patients with chronic diseases mainly featuring cognitive impairment). The data covers the information needed for constructing health portraits and early intervention plans. Use R software and SPSS to analyze, save, and manage the data.
