总体目标:基于节律系统对于代谢稳态的重塑和调节作用,构建糖尿病血糖稳态风险预测模型,具体为: (1)通过生物传感器记录睡眠-进食-运动节律与血糖波动,结合大型生物数据库信息,动态描记节律-血糖共振曲线并阐明中医辨证分型;通过血液生化检测及蛋白组学、代谢组学等多组学联合分析,揭示节律系统驱动代谢稳态的生物学基础。 (2)集成节律信息、血液生化及多组学参数、中医四诊、代谢指标等信息,构建多维动态演化数据库,通过时间序列自组织机器学习技术,构建糖尿病前期节律-血糖稳态风险预测模型,捕捉可预测“未病-已病”状态的特征指标,并生成个体化调节血糖稳态的节律生活模式。 (3)建立肥胖人群横断面研究,验证节律-血糖稳态风险预警模型的敏感性与特异性,比较节律性生活方式与常规生活方式对于调节肥胖人群胰岛素敏感性及代谢稳态的差异。 |

Objectives of Study: |
Overall goal: Based on the remodelling and regulation of metabolic homeostasis by the rhythmic system, to construct a prediction model for diabetic blood glucose homeostasis risk, specifically: (1) Record sleep-feeding-exercise rhythms and blood glucose fluctuations through biosensors, combine with information from a large biological database to dynamically record rhythm-blood glucose resonance curves and elucidate Chinese medicine diagnostic typing; through blood biochemistry and multi-histology analysis, such as proteomics and metabolomics, to reveal the biological basis of the metabolic homeostasis driven by the rhythmic system. (2) Integrate rhythm information, blood biochemistry and multi-omics parameters, the four diagnostic methods of Chinese medicine and metabolic indexes to build a multi-dimensional dynamic evolution database, and construct a prediction model of pre-diabetic rhythm-glycemic homeostasis risk through time-series self-organised machine learning technology, capturing the characteristic indexes for predicting the state of ‘not yet diseased-ready to be diseased’, and generating The model captures the characteristic indicators that can predict the ‘undiseased-already diseased’ state, and generates a rhythmic life pattern for individualised regulation of blood glucose homeostasis. (3) Establish a cross-sectional study of the obese population to verify the sensitivity and specificity of the rhythm-glycemic homeostasis risk prediction model, and compare the differences between rhythmic lifestyle and conventional lifestyle in regulating insulin sensitivity and metabolic homeostasis in the obese population. |

排除标准: |
1) 筛查前1个月内规律服用过影响体重和血糖的药物如降糖药、糖皮质激素、烟酸、β-肾上腺素能激动剂、甲状腺激素、避孕药、二氮嗪、利尿剂、苯妥英钠、γ-干扰素、司美格鲁肽等; 2) 合并各种可能明显缩短预期寿命的疾病,如恶性肿瘤、各脏器功能严重衰竭等; 3) 合并各种可能严重影响受试者参与本试验的疾病,如精神病、严重运动系统异常等; 4) 合并ALT或AST>正常值上限2.5倍,或血肌酐>1.5 mg/dl; 5) 妊娠或哺乳期妇女,或研究期间有妊娠计划的女性; 6) 合并炎症性肠病、肠易激综合征等原因导致的慢性腹泻; 7) 研究者认为不宜纳入本试验者。 |

Exclusion criteria: |
1) Regular use of drugs affecting body weight and blood glucose such as hypoglycaemic agents, glucocorticoids, niacin, β-adrenergic agonists, thyroid hormones, contraceptive pills, diazepam, diuretics, phenytoin sodium, gamma-interferon, simethicone, etc., in the 1 month prior to screening; 2) Combination of various diseases that may significantly shorten life expectancy, such as malignant tumours, severe failure of various organ functions, etc; 3) Combination of various diseases that may seriously affect the subject's participation in this trial, such as psychosis, severe motor system abnormalities, etc; 4) Combination of ALT or AST > 2.5 times the upper limit of normal value, or blood creatinine > 1.5 mg/dl; 5) Pregnant or breastfeeding women, or women with pregnancy plans during the study period; 6) Chronic diarrhoea due to a combination of inflammatory bowel disease and irritable bowel syndrome; 7) those who, in the opinion of the investigator, should not be included in this trial. |
