证明基于多基因甲基化标志物的序贯筛查方式用于上消化道癌筛查的检出率非劣效于传统内镜筛查方式。  

Objectives of Study：

It is proved that the detection rate of the sequential screening method based on multi-gene methylation markers for upper gastrointestinal cancer screening is not inferior to that of the traditional endoscopic screening method.

纳入标准：

Inclusion criteria

排除标准：

1.1年内接受过上消化道内镜筛查； 2.合并严重疾病导致筛查获益降低，如严重的肺部疾病、肾病、肝病、心脑血管及血液疾病等； 3.其他的医生评估为内镜筛查风险过大（如血流动力学不稳定）或不获益（预期寿命短）的情况； 4.孕妇或哺乳期女性； 5.无法签署知情同意书。

Exclusion criteria：

1. Undergo upper gastrointestinal endoscopy screening within 1.1 years; 2. The screening benefits are reduced due to severe diseases, such as severe lung diseases, kidney diseases, liver diseases, cardiovascular and cerebrovascular diseases, and blood diseases, etc. 3. Other doctors assessed that the patient was at excessive risk for endoscopic screening (such as unstable hemodynamics) or had no benefit (short expected lifespan); 4.Pregnant or lactating women; 5.Unable to sign the informed consent form.

统计人员将生成一个随机分配程序，将上消化道癌高风险地区和非高风险地区的群组将以1:1的比例随机分配到传统筛查组或序贯筛查组。分配结果以及每个群组内45-74岁户籍居民的完整名单将分发给每个群组筛查招募人员。 由于传统筛查组和序贯筛查组的干预措施可以明确区分，因此无需对参与者和工作人员进行随机分组。本研究为开放标签研究，受试者和随访人员不设盲，内镜检查医生和统计人员对分组不知情。

Randomization Procedure (please state who generates the random number sequence and by what method)：

Statisticians will generate a random allocation procedure, and groups from high-risk areas and non-high-risk areas of upper gastrointestinal cancer will be randomly assigned in a 1:1 ratio to either the traditional screening group or the sequential screening group. The allocation results and the complete list of registered residents aged 45-74 within each group will be distributed to the screening recruiters of each group. Since the intervention measures of the traditional screening group and the sequential screening group can be clearly distinguished, there is no need to randomly group the participants and staff. This study was an open-label study. The subjects and follow-up personnel were not blinded, and the endoscopists and statisticians were unaware of the groupings.
