研究目的：

本研究依据体位改变对ARDS患者的肺通气及呼吸力学等方面的积极作用，将坐位、半卧位等体位与自主呼吸试验相结合，并利用EIT及膈肌超声监测肺通气及其分布与膈肌功能。探讨体位改变，如坐位、半卧位等对机械通气的ARDS患者自主呼吸试验的影响，及通气分布与各级功能的变化；进一步分析在EIT监测下预测ARDS患者SBT和/或撤机成功或失败的临床结局及其相关危险因素。  

Objectives of Study：

According to the positive effect of posture change on pulmonary ventilation and respiratory mechanics of ARDS patients, this study combined sitting and semi-recumbent posture with spontaneous breathing test, and monitored pulmonary ventilation and its distribution and diaphragm function by EIT and diaphragm ultrasound. To explore the influence of body position changes, such as sitting position and semi-lying position, on spontaneous breathing test of ARDS patients with mechanical ventilation, and the changes of ventilation distribution and functions at all levels; To further analyze the clinical outcome and related risk factors of predicting the success or failure of SBT and/or weaning in ARDS patients under EIT monitoring.

纳入标准：

Inclusion criteria

排除标准：

1. 年龄＜18岁； 2. 妊娠及严重肥胖； 3. 存在肋骨骨折、胸部手等EIT禁忌证； 4. 住ICU时间≤48小时。

Exclusion criteria：

1. Age < 18 years old; 2. Pregnancy and severe obesity; 3. There are contraindications for EIT such as rib fracture, chest and hand; 4. ICU stay time <=48 hours.
