数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

基于病例记录表的数据采集：研究者将患者基本信息（如姓名、年龄、性别、联系方式等）、症状表现、检查检验结果、治疗过程、医嘱等内容，直接在病例记录表上填写相关信息，填写完成后，由另一名研究者对填写的内容进行核对，确保信息的准确性和完整性，避免遗漏重要信息或出现错误，并录入电子数据，备份保存。 数据管理：按照科室、就诊时间、疾病类型等不同维度对病例记录表进行分类，以便于查找和管理，将病例记录表存放在专门的病案库房或文件柜中，采取防火、防潮、防虫等措施，确保病例的安全保存。同时，会为每个病例分配唯一的编号，建立索引目录，方便快速定位和检索。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Data collection based on case record forms: Researchers fill in relevant information directly on the case record forms for patient basic information (such as name, age, gender, contact information, etc.), symptom manifestations, examination and test results, treatment processes, and medical orders. After completion, another researcher checks the filled content to ensure the accuracy and completeness of the information, avoid omitting important information or making errors, and enter electronic data for backup and preservation. Data management: Classify case record forms according to different dimensions such as departments, visit times, and disease types for easy searching and management. Store case record forms in a dedicated medical record warehouse or file cabinet, and take measures such as fire prevention, moisture prevention, and insect prevention to ensure the safe preservation of cases. At the same time, a unique number will be assigned to each case, and an index directory will be established for convenient and rapid location and retrieval.
