Objectives of Study：

To investigate major difficulties and dilemmas in falls prevention based on practice guidelines from the perspectives of hospital implementation and patients' needs, and to propose strategic solutions.

Inclusion criteria

排除标准：

1. 因各种原因（如严重认知功能障碍、失能等）无法配合完成测评、访谈及随访；2. 预期生存时间小于1年。

Exclusion criteria：

1. Unable to complete assessments, interviews, or follow‑up due to various reasons (e.g., severe cognitive impairment, disability, etc.); 2. Expected life expectancy of less than one year.

使用“京颐汇”老年综合评估系统收集数据。数据双人录入，所有原始数据由北京医院进行保存并做到真实性可溯源， 数据进入分析单元时进行脱敏处理，保证患者信息安全。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Data is collected using the "Jingyihui Comprehensive Geriatric Assessment System". All original data undergo double-data entry and are preserved by Beijing Hospital with traceable authenticity. When entering the analysis stage, the data are de-identified to ensure patient information security.
