了解手术切口远程管理系统研究及应用现状，调查患者及家属、医护人员使用意愿及功能需求，开发适用于外科手术切口智能管理的系统，验证切口远程智能管理可行性和有效性，实现手术切口远程管理系统临床应用。  

Objectives of Study：

To understand the research and application status of the surgical wound remote management system, investigate the willingness and functional needs of patients, their families and medical staff, develop a system suitable for the intelligent management of surgical wounds, verify the feasibility and effectiveness of remote intelligent management of surgical wounds, and realize the clinical application of the surgical wound remote management system.

纳入标准：

Inclusion criteria

排除标准：

①严重器官衰竭、其他免疫抑制性疾病或研究者认为不宜参加本临床研究；②非外科手术因素导致的切口图像；③合并手术切口以外伤口，如压力性损伤等。

Exclusion criteria：

1. Severe organ failure, other immunosuppressive diseases, or conditions deemed unsuitable for participation by the investigator; 2. Incision images caused by non-surgical factors; 3. Concurrent wounds unrelated to the surgical incision, such as pressure injuries.

出院前，对患者及家属进行使用培训，在此期间协助完成知情同意，发放伤口智护使用说明书。出院当日，完成首次文字信息填写及图像上传，收集基线信息，建立患者健康档案。根据临床实践指南，设定随访时间为术后30天。使用时间为更换敷料或手术切口感觉异常时，自主报告结构化伤口信息及图像。医护通过App获取患者报告信息的进行异常识别，同时开展线上咨询及自助挂号。如监测到患者超过7日未进行记录，提醒患者查看伤口敷料情况。信息推送按照关节外科、脊柱外科手术不同特征个性化设置。

Intervention：

Before discharge, patients and their families will be trained in the use of the drug, during which they will assist in completing informed consent and distribute instructions for intelligent wound care. On the day of discharge, complete the first text information filling and image uploading, collect baseline information, and establish a patient's health file. According to clinical practice guidelines, the follow-up time was set to be 30 days postoperatively. The time of use is to self-report structured wound information and images when changing dressings or when paresthesia is at the surgical incision. Medical staff obtain patient report information through the App for abnormal identification, and at the same time carry out online consultation and self-service registration. If the patient is monitored for more than 7 days and is not recorded, the patient is reminded to check the wound dressing. The information push is personalized according to the different characteristics of joint surgery and spine surgery.

出院前，发放出院证并告知切口注意事项——根据手术类型不同，隔日、每2~3日，或根据伤口敷料渗血渗液、污染、打湿等情况，前往有资质医疗服务场所，更换伤口敷料，拆线至伤口恢复。如有引流管，则在拔除后7～10日拆线。 出院后，3天电话随访，2-3周门诊随访。

Before discharge, a discharge certificate will be issued and the incision precautions will be informed - according to the type of surgery, every other day, every 2~3 days, or according to the bleeding and oozing, pollution, wetting and other conditions of the wound dressing, go to a qualified medical service place, change the wound dressing, and remove the sutures until the wound recovers. If there is a drainage tube, the sutures will be removed 7~10 days after removal. After discharge, 3-day telephone follow-up and 2-3 weeks outpatient follow-up.

数据采集：问卷、调查表等文字信息由研究人员利用智能手机进行电子问卷发放与收集；手术伤口图像采集使用自带摄像头的智能手机由研究人员及患者上传。 数据管理：文字信息及时导出并储存于研究者电脑中；手术切口图像存储于专用手机相册，同时采用病例记录表，记录患者切口信息、手术切口随访过程中存在的问题及干预前后问卷结果。使用密码或指纹解锁等方式保护设备和相册的访问。
