本课题依托已建立的90多万例全国性多中心自然人群和糖尿病队列，旨在突破当前糖尿病环境因素研究的瓶颈，通过整合临床信息和环境因素，构建动态更新的环境-糖尿病队列研究大数据平台。

Objectives of Study：

This project builds upon an established database of over 900,000 cases from national multicenter natural population and diabetes cohorts. It aims to overcome current bottlenecks in diabetes environmental research by integrating clinical data and environmental factors, thereby constructing a dynamically updated big data platform for environment-diabetes cohort studies.

Inclusion criteria

排除标准：

1.无法提供知情同意：包括认知障碍、意识不清或无法定监护人签署同意书者； 2.I型糖尿病（T1DM）：基于临床诊断（胰岛素依赖起病+酮症倾向）及实验室检查（谷氨酸脱羧酶抗体/GAD-Ab阳性）确诊或怀疑为T1DM的受试者； 3.妊娠期糖尿病（GDM）：妊娠期间首次诊断的高血糖状态； 4.继发性糖尿病：胰腺切除术后、库欣综合征、其他明确继发因素（如长期糖皮质激素使用、血色病等）； 5.严重合并症患者： ①终末期肾病：估算肾小球滤过率（eGFR）<15 ml/min/1.73m^2； ②恶性肿瘤活动期：正在接受放化疗或靶向治疗（缓解期>=5年者除外）； ③精神疾病无法配合随访：包括重度抑郁、精神分裂症等导致无法完成问卷或检查的疾病。

Exclusion criteria：

1. Unable to provide informed consent: including those with cognitive impairment, unconsciousness, or no guardian to sign the consent form; 2. Type I diabetes mellitus (T1DM): subjects with confirmed or suspected T1DM based on clinical diagnosis (insulin-dependent onset ketosis tendency) and laboratory tests (glutamate decarboxylase antibody/GAD-Ab positive); 3. Gestational diabetes mellitus (GDM): hyperglycemic status first diagnosed during pregnancy; 4. Secondary diabetes: after pancreatectomy, Cushing's syndrome, other clear secondary factors (such as long-term glucocorticoid use, hemochromatosis, etc.); 5. Patients with severe comorbidities: (1) End-stage renal disease: estimated glomerular filtration rate (eGFR) <15 ml/min/1.73m^2; (2) Active stage of malignant tumor: receiving chemoradiotherapy or targeted therapy (except for those in remission period >=5 years); (3) Mental illness that cannot cooperate with follow-up: including major depression, schizophrenia and other diseases that prevent the completion of questionnaires or examinations.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：	本研究采用标准化的数据采集与管理体系，包括纸质病例报告表（Case Report Form, CRF）与电子数据采集系统（Electronic Data Capture, EDC）双轨同步。所有研究数据均由经培训的专职研究人员录入，系统设置逻辑核查规则、缺失值提示及异常值报警机制，确保数据准确性与完整性。电子系统基于统一平台部署，支持多中心协同、数据实时上传及远程质控，自动生成数据备份，确保信息安全。所有数据采集内容包括：人口学资料、临床测量、实验室指标、环境暴露参数及生物样本信息等。敏感信息将以匿名编码方式处理，研究数据仅限授权人员访问，并严格按照国家相关法律法规进行管理。
Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：	This study adopts a standardized data collection and management system, including the dual-track synchronization of paper case report form (CRF) and electronic data capture (EDC). All research data are entered by trained full-time researchers, and the system is set up with logical verification rules, missing value prompts and outlier value alarm mechanisms to ensure data accuracy and completeness. The electronic system is deployed based on a unified platform, supporting multi-center collaboration, real-time data upload and remote quality control, automatically generating data backup, and ensuring information security. All data collection includes: demographic data, clinical measurements, laboratory indicators, environmental exposure parameters and biological sample information. Sensitive information will be handled in an anonymous coding manner, and research data will be accessed only by authorized personnel and managed in strict accordance with relevant national laws and regulations.
