本研究旨在评估为期三个月、一周三次的太极拳干预对减轻养老院人群肌少症症状的效果。  

Objectives of Study:

This study aims to evaluate the effects of a three-month, triweekly Tai Chi intervention on reducing sarcopenic symptoms in nursing home residents.

Inclusion criteria

排除标准:

排除标准如下 (1) 目前有感染迹象;(2) 髋关节或膝关节置换;(3) 中度至重度认知障碍;(4) 过去 6 个月内接受过妨碍身体活动的手术;(5) 髋关节骨折史;(6) 乙醇摄入量超过 60 克/天;(7) 有碍行动的严重精神疾病;(8) 无法控制的高血压、心力衰竭、神经肌肉疾病、心血管疾病或肺部疾病。

Exclusion criteria:

The exclusion criteria are as follows: (1) current signs of infection, (2) hip or knee replacement, (3) moderate-to-severe cognitive impairment, (4) surgery in the past six months that hinder physical activity, (5) a history of hip fracture, (6) ethanol consumption exceeding 60 g/d, (7) serious mental illness impairing mobility, (8) uncontrollable hypertension, heart failure, neuro-muscular diseases, cardiovascular disease, or pulmonary illness.

Performing Tai Chi is safe for all participants, and the exercise intensity is comparable to light-to-moderate exercise (3 - 4 METs). In addition, participants will be required to wear a heart rate monitor during the intervention, ensuring a maximum heart rate of 57% - 76% at light-to-moderate exercise levels. Tai Chi exercise consists of multiple moves that can be intricate to perform. Thus, we will select Tai Chi movements for performing traditional Tai Chi breathing and body movements based on the Bafa Wubu of Tai Chi. Each Tai Chi session will last 60 minutes, with 10-min warm-up exercises, 40-min of Tai Chi practice, and 10-min of relaxation, occurring 3 times a week for 12 weeks. The core exercise of 40-minute Tai Chi will include eight hand techniques and five footwork. At the beginning of the intervention, four highly qualified Tai Chi coaches will elucidate and show the principles, practice techniques, and safety considerations associated with each movement. All four coaches will promptly and efficiently identify and rectify improper movements while ensuring safety during intervention in group setting (8-12 participants per group). The first 4 weeks will be the familiarization period to adapt to the forms of Tai Chi intervention. Participants will be encouraged to attend 80% of classes to ensure adherence to the study. Absenting from a session, a reminder will be received to participants by telephone.

Participants in the control group will attend a 12-week health education session in small groups led by research personnel. To match the duration of the Tai Chi session, research personnel will deliver one 60-minute health education class every 2 weeks. The education class primarily focuses on the topics of sarcopenia definition, screening and diagnosis, physical activity prescription, protein supplementation, and vitamin D supplementation. During the intervention period, health education information will also be provided to the Tai Chi group by leaflets/handouts.
