Objectives of Study: |
Multimodal Imaging Characterization To comprehensively evaluate neuroimaging features of cerebral small vessel disease (CSVD) and cerebral hemodynamic regulation in Parkinson’s disease (PD) patients, utilizing a multimodal imaging strategy (e.g., MRI, PET, ASL). Association Analysis for Cognitive Subtyping To identify distinct correlation patterns between specific CSVD factors and domain-specific cognitive impairments (e.g., executive function, memory) through rigorous univariate and multivariate analyses, thereby proposing novel perspectives for PD cognitive impairment subtyping. Mechanistic Exploration via Radiomics & Molecular Biology To investigate the mediating roles of neuroinflammation and oxidative stress in the microvascular-cognitive axis by integrating radiomics and molecular biology approaches (e.g., CSF biomarkers, transcriptomics), uncovering potential therapeutic targets. |

排除标准: |
1.患者有其他伴有步态障碍的疾病(如脊髓关节损伤、肌肉痉挛、卒中、周围神经病变、肌肉疾病、脑积水或认知障碍);患有恶性肿瘤、病情不稳定或严重的内科疾病; 2.患者患有其他类型的帕金森综合征,包括继发性帕金森综合征、帕金森综合征叠加综合征或家族性帕金森综合征; 3.经历过器官衰竭(如心脏、肺、肝脏或肾脏);既往有脑外伤、颅内肿瘤、脑炎、颅内高压或大出血倾向者、癫痫发作史等其他神经系统疾病病史的患者;患有神经精神疾病(如精神分裂症、严重焦虑或抑郁)或不合作的行为;因卒中、脑肿瘤、脑积水或其他原因导致的认知障碍; 4.既往有深部脑刺激(DBS)或消融手术史,或安装心脏起搏器、脑起搏器者、体内有金属植入物者; 5.检测前曾服用影响自主神经功能活性的药物; 6.患者有严重视力障碍,听力障碍及严重痴呆等不能配合认知功能评估的患者。 7.有其他严重疾病,预期生存期不足1年者。 |

Exclusion criteria: |
1. Comorbid Neurological/Motor Disorders:Concurrent gait-affecting conditions (e.g., spinal cord/joint injuries, muscle spasms, stroke, peripheral neuropathy, myopathies, hydrocephalus, or cognitive impairment). Malignancies, unstable/severe systemic diseases. 2. Atypical Parkinsonian Syndromes Secondary parkinsonism, parkinsonism-plus syndromes (e.g., MSA, PSP), or familial Parkinson’s disease. 3. Organ Failure & Neurological/ Psychiatric History;History of organ failure (cardiac, pulmonary, hepatic, renal), brain trauma, intracranial tumors, encephalitis, intracranial hypertension, bleeding diathesis, or epilepsy.Neuropsychiatric disorders (e.g., schizophrenia, severe anxiety/depression) or uncooperative behavior.Cognitive impairment due to stroke, brain tumors, hydrocephalus, etc.Surgical/Device Contraindications 4. Prior deep brain stimulation (DBS), ablation surgery, or implanted devices (pacemakers, neurostimulators, metal implants). 5. Pharmacological & Functional Limitations. Recent use of autonomic nervous system-affecting medications. 6. Severe visual/hearing impairment or dementia precluding cognitive assessments. 7. Severe comorbidities with <1-year survival prognosis. |
