This Research aims to evaluate the effectiveness of a community-based integrated frailty intervention in preventing and controlling frailty among the community-dwelling older adults, using a cluster randomized controlled trial.

干预措施：

干预组接受为期6个月的社区衰弱防控干预。干预期前4个月内参加每周一次、每次约60-90分钟的小组活动（包括衰弱科普健康讲座、锻炼和饮食指导、生活技能提升培训、心理认知训练、慢性病管理指导、健康咨询等健康服务内容）。后2个月内进行线上健康打卡，同时接受每周一次的电话随访、了解并鼓励参与者维持衰弱管理行为。

干预措施代码：

Intervention：

The intervention group received a 6-month community-based intervention to prevent and control frailty. In the first 4 months of the intervention period, they participated in group activities once a week for about 60-90 minutes each time (including frailty knowledge, exercise and diet guidance, life skills, psychological and cognitive training, chronic disease management, health consultation and other health services). In the next 2 months, they checked in online for health and received weekly phone follow-up to track and encourage them to maintain frailty management behaviors.
