本项目旨在开发一项心脏护理系统（CardioCare System），通过在健康检查中心开展随机对照预试验（pilot RCT）以初步评估系统的可行性和潜在有效性，并评估CardioCare在临床或初级保健系统中的成本效益。  

Objectives of Study：

To develop and assess the feasibility and potential cost-effectiveness of the CardioCare System in health check-up centers through a pilot randomized controlled trial (RCT).

纳入标准：

Inclusion criteria

排除标准：

1.先前诊断患有心血管疾病； 2.患有严重疾病，无法参加定期随访评估或遵守研究方案（例如晚期癌症、严重认知障碍）； 3.由医疗保健人员者确定的其预期寿命少于一年； 4.不居住在广州且在研究期间不太可能留在该地区； 5.先前参加过类似的心血管疾病管理计划或研究，可能会影响本研究的结果。

Exclusion criteria：

Exclusion criteria includes participants: 1. with previously diagnosed cardiovascular disease; 2. with severe comorbid conditions that would prevent them from participating in regular follow-up assessments or adhering to the study protocol (e.g., advanced cancer, severe cognitive impairment); 3. with a life expectancy of less than one year, as determined by their healthcare provider; 4. who do not reside in Guangzhou and are unlikely to remain in the area for the duration of the study; 5. who have previously participated in similar CVD management programs or studies that might influence the outcomes of the current study.
