本研究通过一项三臂平行随机对照试验,比较低、中、高三档干扰强度的运动视频游戏对轻度认知障碍(MCI)老年人执行功能的即刻和干预结束后效应。自变量为干扰强度(干扰刺激呈现频率),主要因变量为执行功能表现。次要目标包括MCI相关症状、干预依从性、安全性与参与体验。本研究旨在为基于认知参与剂量的精准化MCI早期干预提供循证依据,推动运动视频游戏在认知增强领域的科学应用。  

Objectives of Study:

This study will use a three-arm parallel randomized controlled trial to compare the immediate and post-intervention effects of low, moderate, and high levels of distraction intensity in exergames on executive function in older adults with MCI. The independent variable is distraction intensity, operationalized by the frequency of interfering stimuli; the primary outcome is executive function performance. Secondary objectives include evaluating MCI-related symptoms, intervention adherence, safety, and user experience. The findings aim to provide empirical evidence for personalized, dose-controlled cognitive engagement strategies in early MCI intervention, supporting the scientific advancement of exergaming for cognitive enhancement.

纳入标准:

Inclusion criteria

排除标准:

不稳定的医学状况或临终疾病;被诊断为痴呆或严重精神疾病(如精神分裂症);严重影响运动能力的感官障碍;静息心率 ≤ 50 次/分(因心律失常)或 ≥ 100 次/分;老年抑郁量表-15项(GDS-15)得分 > 6(Almeida & Almeida, 1999);根据美国运动医学会(ACSM, 2013)指南存在运动禁忌症;当前正在参与其他干预研究。

Exclusion criteria:

1 Unstable medical conditions or terminal illness; 2 Diagnosis of dementia or severe mental illness (e.g. schizophrenia); 3 Severe sensory impairments affecting mobility; 4 Resting heart rate <= 50 bpm due to arrhythmia or >= 100 bpm; 5 A score greater than 6 on the Geriatric Depression Scale-15 (GDS-15); 6 Contraindications to exercise based on the American College of Sports Medicine (ACSM) guidelines (2013); 7 Participation in another ongoing intervention study.

Randomization Procedure (please state who generates the random number sequence and by what method):

Eligible participants will be randomly assigned in a 1:1:1 ratio to one of three parallel groups (low, moderate, or high cognitive engagement) using computer-generated block randomization (block size = 6). Randomization will be stratified by baseline MoCA scores (≤22 vs. >22) to ensure balance in cognitive status across groups.

盲法:

Blinding:

A partial blinding strategy will be employed to minimize bias while maintaining procedural feasibility. Intervention facilitators will be explicitly instructed not to discuss group assignments with participants. During data collection, participants will be instructed not to reveal any details of their assigned intervention to the assessors. All data analysts will remain blinded to group allocation throughout the statistical analysis to ensure objective interpretation of the results.

数据采集和管理(说明:数据采集和管理由两部分组成,一为病例记录表(Case Record Form, CRF),二为电子采集和管理系统(Electronic Data Capture, EDC),如ResMan即为一种基于互联网的EDC:

数据将在三个时间点进行采集:基线、首次干预后和干预完成后。主要结局变量为执行功能,测量工具包括Stroop、Flanker、N-back任务。为保证数据采集的盲法性,数据采集人员将不被告知受试者分组信息,也被禁止与受试者进行除了数据采集之外的接触。测试将在实验室的专用评估室,依据统一标准执行。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture:

Data will be collected at three time points: baseline, after the first intervention, and upon completion of the intervention. The primary outcome variable is executive function, measured using tasks including the Stroop, Flanker, and N-back. To ensure blinding during data collection, assessors will not be informed of participants' group assignments and will be prohibited from interacting with participants beyond the scope of data collection. All assessments will be conducted in a dedicated laboratory assessment room, following standardized procedures.
