Objectives of Study：

This study was designed to assess the efficacy, adverse symptoms, and acceptability between prucalopride 2 mg plus a single dose of 45-mL sodium phosphate and 90-mL in split doses in patients undergoing elective colonoscopy.

药物成份或治疗方案详述：

在大腸鏡檢查前進行有效完整的清腸是重要的，因清腸程度的完整與否與大腸鏡檢查的品 質息息相關。一個好的清腸品質必須使大腸息肉或腫瘤能清楚容易地被檢查者發現。眾所周知， 鏡檢前的清腸是個令人感到不愉快的過程，而這經驗會影響受檢者往後是否願意再接受大腸鏡 追蹤的意願。口服的磷酸鈉溶液(商品名:Fleet)是一種目前市面上廣泛使用的大腸鏡檢查清腸藥 劑，由於它的高滲透壓特性與相比其他清腸藥物較少量的溶液體積，使其清腸效能與耐受性皆 較其他清腸藥物為佳。在沒有慢性腎功能不全或電解質失衡的一般受檢者，他們僅需在大腸鏡 檢查日前一天的傍晚服用一瓶45ml 的磷酸鈉溶液，受檢當日再服用另一瓶相同藥劑，就可以達 到比一般清腸藥劑較有效的清腸品質。但它仍常引起服用者一些如噁心、嘔吐、與腸胃不適的 副作用。Prucalopride 是一種具選擇性的高親和力血清素(5-HT4)受體致效劑，其會透過刺激血 清素5-HT4 受體的作用而誘發巨大移行性的收縮運動，刺激近端結腸蠕動，因此其在治療一些 如便祕、手術後腸阻塞等的腸子蠕動機能異常是有效的。而一天服用一次2mg 的Prucalopride 已證明可改善慢性便秘病人的腸道症狀。 本實驗的目的，我們希望比較服用一顆prucalopride 2mg 與一瓶45ml 磷酸鈉溶液與目前廣 泛使用的兩次劑量磷酸鈉溶液清腸方式在大腸清潔度、受試者耐受性與接受度方面是否有所不 同。我們將收集約200 名門診受試者來接受大腸鏡檢查，所有受試者將隨機分成接受prucalopride 2mg 與一瓶磷酸鈉溶液清腸，或是接受兩瓶的磷酸鈉溶液兩組。一份標準的設計問卷將讓所有 受試者完成。而大腸清腸程度將由內視鏡檢醫師客觀依據清腸程度量化表予以評估。我們假設 兩組別的清腸程度將會是相當的，但在受試者的耐受性與接受度方面，prucalopride 加上磷酸鈉 溶液將比傳統的兩瓶磷酸鈉溶液來的優良許多。 

Description for medicine or protocol of treatment in detail：

Adequate bowel preparation is important in colonoscopy due to its impact on the quality of the procedure. Bowel preparation is the most unpleasant part of the colonoscopy examination, and may influence the willing to have future follow-up examination. The quality of bowel cleansing should be excellent to facilitate detecting neoplastic lesions. Oral sodium phosphate (Fleet) solution is a low-volume, hyperosmotic agent used as part of a colorectal-cleansing preparation for endoscopic examination. Its efficacy and tolerability was generally better than that of PEG or other colorectal cleansing regimens in patients preparing for colonoscopy. A regimen that administered the first dose of sodium phosphate (45mL) on the previous evening and a second dose (45 mL) on the morning of the procedure (10-12 hours apart) was significantly more effective than other colorectal cleansing in preparation for colonoscopy in subjects without significant comorbidities such as renal insufficiency or electrolyte imbalance. However, its adverse events are gastrointestinal symptoms including nausea, vomiting, and abdominal discomfort. Prucalopride, a high-affinity serotonin 5-hydroxytryptamine 4 (5-HT4) receptor agonist, is a novel enterokinetic agent with excellent efficacy, which induces giant migrating contractions of the bowel and stimulates proximal colonic motility by specific and selective stimulation of 5-HT4 receptors. Activation of the 5-HT4 receptor induces facilitation of cholinergic and noncholinergic excitatory neurotransmission, and hence prucalopride has is useful for the treatment of disorders associated with small and/or large bowel dysfunction, including constipation, postoperative ileus, and pseudo-obstruction. Prucalopride 4mg given once daily significantly improves bowel symptoms in patients with chronic constipation. In this study, we aim to compare prucalopride 2 mg with one dose of sodium phosphate (45 mL) with current regimen of 2 separate doses of sodium phosphate (45 mL) regarding the quality of bowel cleaning, and tolerability, and acceptability. We will enroll 200 outpatients, who undergo colonoscopy. All participants will be randomized to receive either two bottles of oral sodium phosphate or prucalopride (2mg) with one bottle of oral sodium phosphate. A standardized questionnaire will be completed by all participants. Bowel cleansing efficacy is assessed based on bowel preparation scale by endoscopists blinded to treatment. We hypothesize that both regimens will be comparable regarding the quality of bowel cleaning, whereas the tolerability and acceptability of bowel cleaning may be better in the group with prucalopride 2mg with one dose of sodium phosphate (45 mL) than conventional regimen.

1.年齡低於20 歲或超過65 以上歲的男女
2.急慢性腎功能不全
3.鬰血性心臟衰竭
4.不穏性心絞痛
5.肝硬化
6,巨腸毒症
7.胃腸阻塞
8.腸穿孔
9.腹部重大手術（如大腸直腸胃, 子宮, 肝臟手術）
10.有輕微，嚴重或是未受控制的精神疾病

Exclusion criteria：

1. Acute and/or chronic renal failure;
2. Congestive heart failure (in any NYHA grade);
3. Unstable angina;
4. Liver cirrhosis;
5. Toxic or congenital megacolon;
6. Intestinal obstruction;
7. Bowel perforation;
8. History of abdominal surgery (involving gastrointestinal tract, uterine, or liver);
9. Psychiatry disorders.
