研究目的：

1.评估在真实临床实践中中，“三明治法”防治重症患者2级IAD的临床疗效，包括皮炎愈合率、愈合时间、皮肤损伤程度改善情况等指标与传统治疗方法的比较。 2.建立“三明治法”防治重症患者2级IAD的临床应用规范和护理流程，为其在临床的广泛推广提供参考。  

Objectives of Study：

1.To evaluate the clinical efficacy of the "sandwich method" in preventing and treating grade 2 IAD in critically ill patients in real clinical practice, including the comparison of indicators such as the healing rate of dermatitis, healing time, and improvement in skin damage severity with traditional treatment methods. 2.Establish the clinical application norms and nursing procedures of the "sandwich method" for preventing and treating grade 2 IAD in critically ill patients, providing a reference for its wide promotion in clinical practice.

排除标准：

1.对乳酸依沙吖定、生长因子及氧化锌的任何一种成分有过敏反应者； 2.患有皮肤性疾病者； 3.各种原因导致皮肤情况不利于观察的患者。

Exclusion criteria：

1. Those who have an allergic reaction to any one of the ingredients of lactate, exaridin, growth factors and zinc oxide; 2. Those suffering from skin diseases; 3. Patients whose skin conditions are not conducive to observation due to various reasons.

按照常规防治措施进行干预，每日及时清理IAD患者的尿液和粪便，按需更换床单、衣物及护理垫。每次温水清洗IAD患处，使用柔软、无刺激性的湿巾和/或棉布按压式清洁皮肤。随后选择团体标准中推荐的皮肤保护用品重组人酸性成纤维细胞生长因子在患处均匀喷涂范围大于尿液、粪便接触的皮肤至少2cm，自然晾干后，用无菌棉签均匀涂抹鞣酸软膏，范围同上。为防止摩擦使用柔软、无刺激棉布隔开患处与衣物。每日晨交班通过失禁性皮炎皮肤损伤评估量表（IAD Severity Instrument，IADS）对患者进行评估，根据分值变化评估干预疗效并判断是否停止干预。

Intervention：

Intervene according to routine preventive measures, cleaning the urine and feces of IAD patients daily in a timely manner, and changing bed sheets, clothes, and care pads as needed. Wash the IAD affected area with warm water each time, using soft, non-irritating wet wipes and/or cotton pads for gentle skin cleansing. Then, apply recombinant human acidic fibroblast growth factor, as recommended by the group standards, to the affected area with a range greater than the skin exposed to urine and feces by at least 2 cm, allowing it to air dry naturally before evenly applying an astringent ointment with a sterile cotton swab, with the same range. To prevent friction, use soft, non-irritating cotton pads to separate the affected area from the clothing. Evaluate patients daily at shift change using the Incontinence-Associated Dermatitis Severity Instrument (IAD Severity Instrument, IADS), assessing the intervention's effectiveness based on score changes and determining whether to stop the intervention.

根据IAD的结构化皮肤管理原则制定“三明治法”干预流程。IAD是一种发病迅速且病情呈动态演变的皮肤病症，其病情进展具有显著的连续性和递进性。为及时应对IAD的病情变化，每日晨交班时，IAD小组成员采用IADS和PAT评分工具评估患者情况，根据IADS分值变化判断是否停止干预；通过PAT评分确定干预频率，该工具从刺激物强度、刺激持续时间、皮肤状况以及相关影响因素4个维度判断IAD风险，总分为4~12分，其中4~6分为低风险，每日干预1次，7~12分为高风险，每日干预2次，间隔8h。每次温水清洁IAD患处后，使用三明治内层-乳酸依沙吖定抗菌：该药物能够减轻炎症反应，降低感染风险。根据皮损面积选择足量无菌纱布（6层），放入乳酸依沙吖定溶液中完全浸湿，然后将纱布拧至不滴液为度，随后将其贴在皮损处，轻轻按压使纱布与皮损紧密贴合，范围大于尿液、粪便接触的皮肤至少2cm，湿敷时间20~30分钟，湿敷后充分待干；三明治中层-生长因子促进愈合：生长因子能够调节细胞的生长、增殖和分化，为皮肤的修复提供结构支持。在患处均匀喷涂外用重组人酸性成纤维细胞生长因子，范围同湿敷面积；三明治外层-氧化锌糊防护：该药物能够有效隔离刺激物，具有收敛的特性。待生长因子在患处表面形成保护膜，用无菌棉签均匀涂抹氧化锌糊，范围同湿敷面积。若用药干预后患者发生大小便失禁并污染氧化锌糊防护层，需清洗后重新涂抹氧化锌糊。

Intervention：

Based on the structured skin management principles of IAD, the "sandwich method" intervention process was formulated. IAD is a skin disorder with rapid onset and dynamic evolution of the condition, and its progression has significant continuity and progressive nature. To promptly respond to the changes in the condition of IAD, on the morning shift report, members of the IAD team evaluated the patient's condition using the IADS and PAT scoring tools, and determined whether to stop the intervention based on the changes in IADS scores; the intervention frequency was determined by the PAT score. This tool assesses the risk of IAD from four dimensions: stimulus intensity, stimulus duration, skin condition, and related influencing factors, with a total score ranging from 4 to 12 points. A score of 4 to 6 indicates low risk, with daily intervention once, 7 to 12 indicates high risk, with daily intervention twice at an 8-hour interval. After each warm water cleaning of the IAD affected area, use the inner layer of the sandwich - clindamycin lactobionate for antibacterial purposes: this drug can alleviate inflammatory reactions and reduce the risk of infection. Select an adequate amount of sterile gauze (6 layers) based on the lesion area, completely immerse it in the clindamycin lactobionate solution, then wring it until no liquid drips, and then apply it to the lesion area, gently pressing to make the gauze closely adhere to the lesion, with a range larger than the skin area contacted by urine and feces by at least 2 cm. The wet compress time is 20 to 30 minutes, and then wait for it to fully dry. The middle layer of the sandwich - growth factor for wound healing: growth factors can regulate cell growth, proliferation, and differentiation, providing structural support for skin repair. Uniformly spray the topical recombinant human acidic fibroblast growth factor on the affected area, with the range the same as the wet compress area; the outer layer of the sandwich - zinc oxide paste for protection: this drug can effectively isolate stimuli and has astringent properties. Once the protective film formed by the growth factor on the lesion surface is complete, apply zinc oxide paste evenly with sterile cotton swabs, with the range the same as the wet compress area. If the patient experiences incontinence of urine and stool and contaminates the zinc oxide paste protective layer after medication intervention, it is necessary to clean and reapply the zinc oxide paste.
