研究目的:

(1) 评估 2 型糖尿病患者基线心血管功能代谢特征 (2) 评估 2 型糖尿病对患者心肺耐力下降的情况及其影响因素 (3) 评估 2 型糖尿病患者血液生物标志物与心脏超声指标、心肺耐力的相关性 (4) 探究运动干预对 2 型糖尿病患者心血管代谢、心功能及血液生物标记物的影响 (5) 探究运动干预对 2 型糖尿病患者心肺耐力及其影响因素的影响 (6) 探究运动干预后生物标志物变化与心肺耐力、心脏超声指标的相关性 (7) 探究介导运动干预改善心肺功能的主要代谢途径  

Objectives of Study:

(1) To assess the baseline cardiovascular function metabolic profile of patients with type 2 diabetes mellitus (2) To assess the impact of type 2 diabetes mellitus on the decline of cardiorespiratory endurance in patients and the factors that influence it. (3) To assess the correlation of blood biomarkers with cardiac ultrasound parameters and cardiorespiratory endurance in patients with type 2 diabetes mellitus. (4) To investigate the effects of exercise intervention on cardiovascular metabolism, cardiac function and blood biomarkers in patients with type 2 diabetes mellitus. (5) To investigate the effects of exercise intervention on cardiorespiratory endurance and its influencing factors in patients with type 2 diabetes mellitus. (6) To investigate the correlation between changes in biomarkers and cardiorespiratory endurance and cardiac ultrasound indices after exercise intervention. (7) To investigate the main metabolic pathways mediating the improvement of cardiopulmonary function by exercise intervention.

纳入标准:

Inclusion criteria

排除标准:

存在肾脏疾病、视网膜病变、神经系统疾病、骨科疾病、肿瘤疾病或肺部疾病而不能进行运动试验者;和/或有心血管疾病(如瓣膜疾病、冠状动脉疾病、先天性心脏病、呼吸困难症状或胸痛;恶性肿瘤或其他主要全身性疾病;1 型糖尿病; HbA 1c> 9.5%或空腹血糖(FPG)> 15.0 mmol/L(270 mg/dL);有糖尿病酮症酸中毒或高血糖高渗状态病史;收缩压(SBP)> 180 mmHg 或舒张压(DBP)> 95 mmHg。

Exclusion criteria:

Presence of renal disease, retinopathy, neurological disease, orthopaedic disease, oncological disease, or pulmonary disease that precludes exercise testing; and/or cardiovascular disease (e.g., valvular disease, coronary artery disease, congenital heart disease, respiratory distress symptoms, or chest pain; malignant neoplasm, or other major systemic disease); type 1 diabetes mellitus; HbA 1c > 9.5% or fasting blood glucose (FPG) > 15.0 mmol/L (270 mg/dL); history of diabetic ketoacidosis or hyperglycaemic hyperosmolar state; systolic blood pressure (SBP) > 180 mmHg or diastolic blood pressure (DBP) > 95 mmHg.

在监督下进行 30 分钟高强度间歇运动,运动过程中受试者佩戴心率带对心率进行监控,高强度间歇运动方案 4×4 分钟高强度(70~90%HRR)间隔 3 分钟低强度(< 50%HRR)恢复,每周运动 3 次,共 12 周;运动前后各包括 5 分钟的动态拉伸和热身。

30 minutes of supervised high-intensity interval exercise, during which subjects wore a heart rate belt to monitor their heart rate, with a high-intensity interval exercise regimen of 4 × 4 minutes of high-intensity (70-90% HRR) with a 3-minute low-intensity (< 50% HRR) recovery interval, 3 times per week for 12 weeks; pre- and post-exercise periods included 5 minutes each of dynamic stretching and warming up.
