研究目的：

（1）基于已有的高危新生儿随访队列中的早产儿临床病历及随访数据库，已经建立回顾性早产儿出生队列；将符合纳入标准的早产儿纳入前瞻性出生队列； （2）针对早产儿出生纵向队列，开展巢式病例对照研究，对学龄期和青春期早产儿进行访视，体格检查，从而了解早产儿学龄期和青春期健康； （3）纵向队列数据库设计成型后，建立前瞻性队列，建立生物样本库，并由此开始纵向队列的长期随访，预期随访至成年期，成为富有三院特色的早产儿出生纵向队列，并为青春期生育力评估和成年期健康状况提供信息数据。  

Objectives of Study：

(1) Based on the clinical medical records of preterm infants and the follow-up database in the existing high-risk neonatal follow-up cohort, a retrospective birth cohort of preterm infants has been established. Preterm infants meeting the inclusion criteria will be included in the prospective birth cohort. (2) For the longitudinal birth cohort of preterm infants, a nested case-control study will be carried out. Visits and physical examinations will be conducted on preterm infants in the school-age period and adolescence to understand their health status during these periods. (3) After the design of the longitudinal cohort database is finalized, a prospective cohort will be established, and a biobank will be set up. Then, long-term follow-up of the longitudinal cohort will be initiated, with the expectation of following up until adulthood. This will become a longitudinal birth cohort of preterm infants with the characteristics of our Hospital, and provide information and data for the assessment of fertility in adolescence and the health status in adulthood.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

拟在已建立的儿童健康发展中心早产儿出生回顾性队列基础上，根据已有随访模式：0~1岁，2个月随访一次，共计6次；1~2次，3个月一次，共计4次，2~3岁6个月一次，共计2次，因此随访至3岁，共计随访12次。队列将结合住院期间的临床数据、随访数据和学龄期、青春期访视体检数据，进行数据库建立。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Based on the established retrospective cohort of premature infants at the Children's Health Development Center and in accordance with the existing follow-up model: 0-1 year old, follow-up is conducted once every 2 months, for a total of 6 times; 1 to 2 times, once every 3 months, for a total of 4 times. 2 to 3 years old, once every 6 months, for a total of 2 times. Therefore, the follow-up was conducted until the age of 3, with a total of 12 follow-ups. The cohort will establish a database by combining clinical data during hospitalization, follow-up data, and visit and physical examination data during the school age and adolescence.
