研究目的:

本研究拟在通过将肌电反馈与镜像疗法及呼吸训练进行结合,在基于镜像的肌电反馈训练的过程中进行呼吸训练。即基于镜像的肌电反馈训练改善患者的功能,呼吸训练缓解患者的疼痛,从而形成一种系统的治疗方法,在多方面调节患者的生理和心理状态。在促进功能恢复的同时,减轻周围神经损伤后引发的疼痛,提升整体治疗效果,进一步提高对于周围神经损伤患者的疗效。  

Objectives of Study:

This study proposes to conduct breathing training during mirror-based EMG feedback training by combining EMG feedback with mirror therapy and breathing training. In other words, mirror-based EMG feedback training improves patients' function and respiratory training relieves patients' pain, thus forming a systematic treatment method that regulates patients' physiological and psychological status in various aspects. In addition to promoting functional recovery, it also reduces the pain caused by peripheral nerve injury, improves the overall therapeutic effect, and further improves the therapeutic efficacy for patients with peripheral nerve injury.

Inclusion criteria

排除标准:

1.骨折未愈合 2.任何会影响患者遵守研究方案能力的心理或医学状况 3.存在大量静脉血栓 4.单侧忽视或偏盲等视知觉障碍者 5.严重高血压(>=2级) 6.有失用症的患者 7.坐床角度<=45° 8.认知、言语功能障碍 9.患者及家属不合作,不能配合完成实验的患者 10.严重的肺部疾病(如肺气肿、重度哮喘)、癫痫、肺结核或其他传染性疾病、易呼吸性酸中毒的患者;

Exclusion criteria:

1. Fracture did not heal 2. Any psychological or medical condition that may affect the patient's ability to adhere to the study protocol 3. The presence of massive venous thrombosis 4. Patients with visual perception disorders such as unilateral neglect or hemianopia 5. Severe hypertension (>= grade 2) 6. Patients with apraxia 7. Bed Angle <=45° 8. Cognitive and speech dysfunction 9. Patients and their families do not cooperate and cannot cooperate to complete the experiment 10. Patients with severe lung diseases (such as emphysema, severe asthma), epilepsy, tuberculosis or other infectious diseases, or prone to respiratory acidosis;

利用好博HB120B肌电反馈电刺激仪,进行肌电反馈干预,电极片放置在需要康复的肌肉处,肌腹与肌腱处各一,持续电刺激30min,阈值根据患者实际耐受进行频率的调整。镜像疗法镜面朝向健侧肢体。该治疗方案患者无法观察到患侧肢体,需通过镜像视觉反馈将镜面中的健侧下肢运动想象为患侧肢体。患者在训练期间根据“收缩”指令(持续10秒)进行最大努力的运动,尽最大可能地产生动作,同时通过镜面观察健侧肢体的运动,并将其想象为患侧肢体的运动表现。在此期间患者的主动肌肉收缩会通过肌电反馈电刺激仪实时监测,并自动将患者引发的肌电阈值同预设阈值进行比较。根据“治疗”指令进行下肢放松并感知有无电刺激。当患者自主引发的肌电振幅超过预设的阈值时,肌电反馈电刺激系统将自动触发电刺激,向患者的患侧肢体施加持续10秒的电刺激。若肌电信号未达到阈值,系统将不会触发电刺激。“收缩”和“治疗”阶段交替训练,每个阶段持续10秒。

Intervention:

Utilizing the Hobo HB120B myoelectric feedback electrical stimulator, myoelectric feedback intervention was performed with electrode pads placed at the muscles to be rehabilitated, one at each of the muscle belly and tendon, with continuous electrical stimulation for 30 min, with thresholds adjusted for frequency according to the patient's actual tolerance. Mirror therapy mirror facing the healthy limb. In this treatment program, the patient cannot observe the affected limb, and needs to visualize the movement of the healthy lower limb in the mirror as the affected limb through mirror visual feedback. During the training period, the patient performs a maximal effort movement according to a "contraction" command (lasting 10 seconds) to maximize movement production, while observing the movement of the healthy limb in the mirror and visualizing it as the movement of the affected limb. During this period, the patient's active muscle contractions are monitored in real time by an EMF stimulator, which automatically compares the patient's triggered EMG thresholds with preset thresholds. The lower limbs are relaxed and the presence or absence of electrical stimulation is sensed according to the "treatment" command. When the patient's autonomously triggered EMG amplitude exceeds the preset threshold, the EMG system will automatically trigger the electrical stimulation, applying electrical stimulation to the patient's affected limb for a duration of 10 seconds. If the EMG signal does not reach the threshold, the system will not trigger the electrical stimulation. The "contraction" and "treatment" phases alternate, each lasting 10 seconds.
