研究目的：

1.构建一个基于患者报告结局的居家癌痛患者管理方案。 2.验证基于云平台的居家癌痛患者管理方案实际临床效果，并予以完善，为改善居家癌痛患者管理提供科学实践工具。

1. Develop a home-based cancer pain patient management program based on patient-reported outcomes (PROs). 2. Validate the program's clinical effectiveness based on a cloud platform, and refine it to provide a scientific and practical tool for improving the management of home-based cancer pain patients.

纳入标准：

Inclusion criteria

排除标准：

①有严重精神障碍者；②肿瘤晚期虚弱患者，无法全程参与配合完成。

Exclusion criteria：

1.Patients with severe mental disorders; 2.Patients with advanced cancer who are too frail to participate and cooperate fully throughout the program.

干预措施：

通过平台向患者手机发送问卷，收集患者疼痛强度、疼痛干扰、身体功能、睡眠相关障碍、焦虑、抑郁、参与社会角色和活动的能力自我报告数据。设定各领域报告阈值，当报告内容超过设定阈值时，短信提醒癌痛管理团队及时进一步评估与处理，癌痛管理团队定期回顾报告结果，并给予电话随访。根据患者报告结果和需求，云随访系统定期推送疼痛知识、疼痛应对技巧、止痛药物使用方法、药物副作用应对等知识；发送服药提醒、随访提醒等，同时电话随访时指导患者积极使用非药物镇痛疗法，如分散注意力、正念疗法、音乐疗法等，以协同加强药物镇痛效果。

Intervention：

Questionnaires are sent to patients' mobile phones via the platform to collect self-reported data on pain intensity, pain interference, physical function, sleep-related disturbances, anxiety, depression, and the ability to participate in social roles and activities. Thresholds for reporting in each domain are set. When the reported content exceeds the set threshold, a text message alert is sent to the cancer pain management team to prompt them to conduct a timely further assessment and intervention. The cancer pain management team regularly reviews the reported results and provides follow-up calls to patients. Based on the patients' reported outcomes and needs, the cloud follow-up system regularly pushes knowledge on pain management, including pain knowledge, pain coping skills, methods of analgesic medication use, and management of drug side effects. It also sends medication and follow-up reminders. During phone follow-ups, patients are encouraged to actively use non-pharmacological analgesic therapies, such as distraction techniques, mindfulness therapy, and music therapy, to enhance the effectiveness of pharmacological pain relief in a complementary manner.

The conventional telephone follow-up method is adopted, which involves follow-up calls at 1 week and 3 weeks after discharge. Nurses in the ward, who have undergone unified training, conduct the follow-ups based on a predetermined follow-up form. The content of the phone calls includes pain intensity and location, medication use, and any adverse drug reactions.
