研究目的：

腹腔镜手术是肝脏外科发展的主流术式。荧光、成像技术辅助腹腔镜手术有助于精准切除肿瘤。申请人团队前期联合迈瑞公司研发了国内第一款多模态融合腔镜手术系统，本课题旨在研究利用自主研发的多模态腹腔镜系统进行解剖性半肝切除术的有效性，建立标准化技术流程，为拓展国产高端外科器械的临床应用及探讨新系统辅助下的腔镜技术的效果提供高级的循证医学证据。本研究目的：对比多模态腹腔镜系统和进口传统腹腔镜系统行半肝切除术的有效性、安全性，建立基于国产多模态腹腔镜系统半肝切除新技术标准。

Objectives of Study：

Laparoscopic surgery has become the mainstream approach in liver surgery. The use of fluorescence and imaging technologies to assist laparoscopic procedures facilitates precise tumor resection. In earlier work, the applicant’s team collaborated with Mindray to develop the first domestic multimodal integrated laparoscopic surgical system. This project aims to evaluate the efficacy of performing anatomical hemihepatectomy using this independently developed multimodal laparoscopic system, establish a standardized surgical workflow, and provide high-level evidence-based medical support for both the clinical application of domestically produced high-end surgical instruments and the evaluation of laparoscopic techniques assisted by the new system.Objective of the Study:To compare the efficacy and safety of hemihepatectomy performed using the multimodal laparoscopic system versus imported traditional laparoscopic systems, and to establish a new technical standard for hemihepatectomy based on the domestic multimodal laparoscopic system.

Inclusion criteria

排除标准：

1）既往肝细胞癌治疗史（如肝细胞癌放化疗，靶免治疗等）； 2）合并肝脏以外的恶性肿瘤病史； 3）病理为非肝细胞癌； 4）临床上有明显门脉高压表现，如食道静脉曲张、脾肿大(>12 cm)、血小板计数<10×10^9/ml； 5）合并其他系统、器官严重疾病，手术禁忌者。

Exclusion criteria：

1.History of prior treatment for hepatocellular carcinoma (e.g., radiotherapy, chemotherapy, targeted or immunotherapy); 2.History of malignancies outside the liver; 3.Non-hepatocellular carcinoma pathology; 4.Clinical evidence of significant portal hypertension, such as esophageal varices, splenomegaly (>12 cm), or platelet count <10×10⁹/mL; 5.Presence of severe comorbidities involving other systems or organs that constitute contraindications to surgery.

由依托单位的临床研究中心的专业统计学人员通过 SAS 系统产生的随机数进行随机分组，受试者按 1:1 的比例，遵循对照、随机分组、分组隐匿等原则，随机分配到传统腹腔镜组和多模态腹腔镜组。

Randomization Procedure (please state who generates the random number sequence and by what method)：

Randomization will be performed by professional statisticians from the Clinical Research Center of the sponsoring institution using random numbers generated by the SAS system. Subjects will be randomly assigned in a 1:1 ratio to either the Conventional Laparoscopy Group or the Multimodal Laparoscopy Group, in accordance with the principles of controlled, randomized allocation and allocation concealment.

盲法：	单盲法。试验中，每个受试者按入院后随机分组的情况定为相应组别，参与手术的医生根据组别决定采用传统腹腔镜半肝切除术和多模态腹腔镜半肝切除术。而病人、数据检测、数据采集和数据分析者均保持盲态。所有随访数据的检测和收集均由临床研究中心的独立人员进行。
Blinding：	Single-blind method. During the trial, each subject will be assigned to the corresponding group based on the randomization outcome after hospital admission. The surgeon will perform either conventional laparoscopic hemihepatectomy or multimodal laparoscopic hemihepatectomy according to the assigned group. Blinding will be maintained for the patients, data assessors, data collectors, and data analysts. All follow-up data will be assessed and collected by independent personnel from the Clinical Research Center.
