研究目的：

支扩合并PA慢性感染易耐药、难治疗、难清除、预后差。现有指南推荐的抗生素长期治疗方案有效率不高，毒副作用大，且有诱导耐药及菌群失调的风险。本项目拟采取自身前后对照研究，通过雾化吸入广谱噬菌体鸡尾酒Pae-PC1治疗支扩合并PA慢性感染，评估噬菌体Pae-PC1的疗效及安全性，并探索噬菌体治疗对支扩患者呼吸道本底噬菌体组、蛋白组、代谢组的影响，为噬菌体治疗支扩合并PA慢性感染提供新的治疗选择。

Objectives of Study：

Bronchiectasis with chronic Pseudomonas aeruginosa (PA) infection is prone to antibiotic resistance, difficult to treat, challenging to eradicate, and associated with poor prognosis. Current guideline-recommended long-term antibiotic regimens demonstrate limited efficacy, significant adverse effects, and carry risks of inducing further resistance and microbial dysbiosis. This self-controlled study aims to evaluate the efficacy and safety of nebulized broad-spectrum phage cocktail Pae-PC1 in treating bronchiectasis patients with chronic PA infection. Additionally, we will investigate the impact of phage therapy on the baseline respiratory phageome, proteome, and metabolome in these patients, thereby providing a novel therapeutic alternative for managing bronchiectasis with chronic PA infection.

Inclusion criteria

排除标准：

1.孕产妇及哺乳期妇女； 2.未能控制的严重支气管哮喘； 3. 神经精神异常等不能表达真实意愿； 4.近3个月抗生素治疗PA后痰培养PA阴性； 5. 既往雾化吸入出现严重气道痉挛，不能耐受等情况； 6.患有不稳定心脑血管疾病，定义为在筛查前 30天现临床恶化症状（如不稳定心绞痛、快速房颤、脑出血、急性脑梗死等）； 7.患有进行性或未能控制的全身性疾病，例如晚期恶性肿瘤、严重呼吸、循环、神经或精神系统的疾病，研究者评估在试验期间上述情况不稳定或可能加重。

Exclusion criteria：

1.Pregnant or lactating women; 2.Uncontrolled severe bronchial asthma; 3.Cognitive or psychiatric impairment affecting the ability to provide informed consent; 4.Negative PA sputum culture after anti-PA antibiotic therapy within the past 3 months; 5.History of severe airway spasm or intolerance to nebulized therapy; 6.Unstable cardiovascular or cerebrovascular diseases, defined as clinical deterioration (e.g., unstable angina, rapid atrial fibrillation, cerebral hemorrhage, acute cerebral infarction) within 30 days prior to screening; 7.Progressive or uncontrolled systemic diseases, including advanced malignancies, or severe respiratory, circulatory, neurological, or psychiatric disorders, as assessed by the investigator to be unstable or likely to worsen during the trial.
