研究目的：

基于此，本研究以我科ICU收治的机械通气老年患者为研究对象，采用主动循环呼吸技术结合音乐疗法，旨在探讨对ICU机械通气老年患者更加有效的脱机拔管的护理方法，为ICU机械通气患者拔管提供借鉴。  

Objectives of Study：

Based on this, this study takes the mechanically ventilated elderly patients admitted to ICU of our department as the research object, and adopts active cycle respiratory technique combined with music therapy, aiming to explore more effective nursing methods of off-board extubation for mechanically ventilated elderly patients in ICU, and to provide reference for extubation of mechanically ventilated patients in ICU.

纳入标准：

Inclusion criteria

排除标准：

1.合并听力障碍、认知功能障碍无法配合训练患者； 2.使用肌松药者； 3.呼吸循环功能衰竭患者； 4.重症肌无力患者； 5.合并慢性阻塞性肺疾病、支气管哮喘患者； 6.多器官功能障碍综合征（MODS）者； 7.正在进行其他临床试验者。

Exclusion criteria：

1. Patients with hearing impairment and cognitive dysfunction who are unable to cooperate with the training; 2. Patients using inotropic drugs; 3. Patients with respiratory and circulatory failure; 4. Patients with myasthenia gravis; 5. Patients with COPD and bronchial asthma; 6. Patients with multiple organ dysfunction syndrome (MODS); 7. Those who are undergoing other clinical trials.

1.术前一日，发放《脱机拔管训练手册》，一对一指导；2.将主动循环呼吸技术音频导入音乐播放器中，让患者感受音频，训练呼吸节律。根据患者自身喜好，挑选5~6首节奏舒缓平稳的音乐曲目，设置相关内容：音量不高于60分贝，播放方式为全部循环模式，音乐时长30分钟，音乐结束后启动闹铃提醒。3.二维码视频示教。

1. One day before the operation, issue the "offline extubation training manual", one-on-one guidance; 2.import the active cycle respiratory technology audio into the music player, so that the patient can feel the audio and train the respiratory rhythm. According to the patient's own preferences, select 5~6 music tracks with soothing and smooth rhythms, set the relevant content: volume not higher than 60 decibels, playback mode for all the cycle mode, music duration 30 minutes, start the alarm to remind after the end of the music. 3.Two-dimensional code video demonstration
