研究目的：

纳入机械通气时长＞48小时的重症患者，观察首次支气管镜检查时气道黏膜及痰液情况，对痰液量、痰液颜色、痰液粘稠度、支气管黏膜水肿、支气管黏膜充血程度进行分级评价，并进行评分。观察不同肺部感染情况患者镜下表现是否存在差异性，评估镜下表现情况与肺部感染发生率的相关性，并分析评分患者预后情况是否存在差异，以评估支气管镜下表现分级对重症患者肺部感染发生及预后的评价价值。  

Objectives of Study：

Critically ill patients with mechanical ventilation for more than 48 hours were enrolled. The airway mucosa and sputum at the first bronchoscopy were observed. The sputum volume, sputum color, sputum viscosity, bronchial mucosal edema, and bronchial mucosal hyperemia were graded and scored. To observe whether there are differences in the endoscopic manifestations of patients with different pulmonary infection, to evaluate the correlation between the endoscopic manifestations and the incidence of pulmonary infection, and to analyze whether there are differences in the prognosis of patients with scoring, so as to evaluate the value of bronchoscopic manifestations grading in the occurrence and prognosis of pulmonary infection in severe patients.

纳入标准：

Inclusion criteria

排除标准：

1. ＜18岁； 2. 孕妇； 3. 气管食管瘘； 4. 肺部近期手术病史； 5. 外伤性肺部损伤。

Exclusion criteria：

1. < 18 years; 2. Pregnant; 3. Tracheoesophageal fistula; 4. Recent pulmonary surgery history; 5. Traumatic lung injury.
