主要目的：评价应用互联网远程数据传输监测患者家庭血压，并对患者进行高血压知识教育，是否能够提高高血压患者的血压控制水平。 次要目的：评价应用互联网远程数据传输监测患者家庭血压，并对患者进行高血压知识教育，是否能降低高血压患者的心血管风险。  

Objectives of Study：

First objective: to evaluate wether the application of internet remote data transmission to monitor patients' home blood pressure and patient education for patients with hypertension knowledge could improve the blood pressure control level of hypertensive patients. Second objective: to evaluate wether the application of internet remote data transmission to monitor patients' home blood pressure and patient education for patients with hypertension knowledge could reduce the cardiovascular risk of hypertensive patients.

纳入标准：

Inclusion criteria

排除标准：

不同意参加该研究的患者；处于妊娠期或计划妊娠的患者；发生过急性冠脉综合症、冠状动脉血运重建或脑卒中的患者；已知原因的继发性高血压患者；其他被认为无法纳入研究的患者。

Exclusion criteria：

Potential participants were excluded if they met any of following criteria: current or planned pregnancy during the study period; patients with acute coronary syndrome, coronary artery revascularization or stroke;secondary hypertensive patients with known causes, and others who are considered unable to be included in the study.
