核心目标: 探究基于传统江南饮食(JNd)和中等强度有氧运动(AEx)的单独及联合干预对NAFLD患者的肝脏脂肪变性、肝纤维化及代谢健康的改善效果。 分项目标: (1) 评估传统江南饮食对NAFLD患者体重、体成分及血脂的调控作用; (2) 分析中等强度有氧运动对肝脏酶学指标、尿酸及血糖的干预效果; (3) 验证饮食-运动联合干预的协同效应,探索其对综合代谢参数和肝功能的优化潜力。  

Objectives of Study:

Core objective: To explore the improvement effects of individual and combined interventions based on traditional Jiangnan diet (JNd) and moderate-intensity aerobic exercise (AEx) on liver steatosis, liver fibrosis and metabolic health in patients with non-alcoholic fatty liver disease (NAFLD). Sub-objectives: (1) To evaluate the regulatory effects of traditional Jiangnan diet on body weight, body composition and blood lipids in patients with NAFLD; (2) To analyze the intervention effects of moderate-intensity aerobic exercise on liver enzymology indicators, uric acid and blood glucose; (3) To verify the synergistic effect of diet-exercise combined intervention and explore its potential for optimizing comprehensive metabolic parameters and liver function.

纳入标准:

Inclusion criteria

排除标准:

(1) 合并存在严重全身性疾病(如肺、肾、代谢等系统疾病或急性传染病)或无法耐受本研究制定的有氧运动量; (2) 存在肝脏疾病进展:包括肝硬化、肝细胞癌等NAFLD进展的明确证据; (3) 存在神经性贪食症、药物滥用、严重抑郁症、精神病治疗等禁忌症; (4) 肿瘤相关因素及甲状腺特定情况:包括转移性病灶(淋巴结转移、腺外侵犯、远处转移)、双侧或峡部甲状腺癌、严重桥本甲状腺炎,以及术中需转换手术类型的情况。

Exclusion criteria:

(1) Combined with serious systemic diseases (such as lung, kidney, metabolic and other system diseases or acute infectious diseases) or unable to tolerate the aerobic exercise prescribed in this study; (2) The presence of liver disease progression: clear evidence of NAFLD progression, including cirrhosis and hepatocellular carcinoma; (3) There are contraindications such as bulimia nervosa, drug abuse, severe depression, and psychiatric treatment; (4) Tumor-related factors and thyroid specific conditions, including metastatic lesions (lymph node metastasis, extrandular invasion, distant metastasis), bilateral or isthmic thyroid cancer, severe Hashimoto thyroiditis, and conditions requiring intraoperative switching of surgical types.

After screening for inclusion and exclusion criteria, subjects were first stratified by sex (male/female) and age group. In each layer, subjects were numbered according to their weight from smallest to largest. Then, the function in SPSS software is used to generate a table of random numbers, and the numbers are sorted in order from smallest to largest. The sorted values were divided by 4, and the remainder was extracted to determine the grouping: subjects with a remainder of 0 were divided into Control group, those with a remainder of 1 were divided into aerobic exercise group (AEx), those with a remainder of 2 were divided into Jiangnan diet group (JNd), and those with a remainder of 3 were divided into Jiangnan diet + aerobic exercise group (JNd+AEx).
