研究目的：

本研究旨在探讨超声骨刀“掀盖法”拔除下颌埋伏阻生智齿后，于拔牙窝内置入医用胶原蛋白海绵，对促进拔牙创骨愈合、修复邻牙（第二磨牙）远中骨缺损的临床效果。通过影像学测量评估，为临床提供一种促进骨愈合、保存颌骨完整性的微创有效方法。  

Objectives of Study：

This study aims to investigate the clinical efficacy of combining the piezoelectric surgery "lid-flap" technique with medical collagen sponge implantation following the extraction of impacted mandibular third molars. The primary focus is to evaluate its potential in promoting alveolar bone healing and regenerating distal bone defects of the adjacent second molar, using cone-beam computed tomography (CBCT) measurements. The goal is to provide a minimally invasive and effective clinical protocol for preserving alveolar bone integrity.

纳入标准：

Inclusion criteria

排除标准：

患有不可控制的心脏病、高血压、血液系统疾病、糖尿病等系统性疾病，或处于月经期的女性；术前有急性炎症病史，或存在活动性牙周炎、严重牙槽骨丧失等口腔健康问题；对药物过敏和胶原蛋白产品过敏的患者；无法配合完成术中手术操作及术后随访评估的患者；有明显吸烟习惯（每天吸烟超过 20 支）的患者，以及妊娠期女性。

Exclusion criteria：

Patients with uncontrolled systemic diseases such as heart disease, hypertension, hematological disorders, and diabetes mellitus; female patients during menstruation; patients with a history of acute inflammation before surgery, or oral health problems including active periodontitis and severe alveolar bone loss; patients allergic to drugs or collagen products; patients unable to cooperate with intraoperative surgical procedures and postoperative follow-up assessments; patients with a significant smoking habit (more than 20 cigarettes per day); and pregnant women.

A random number sequence was generated using the SPSS statistical software by an independent statistician who was not involved in patient recruitment or surgical procedures. The grouping results were then placed in sequentially numbered, light-impermeable sealed envelopes.

盲法：

Blinding：

Blinding Implementation: This study adopted an assessor-blinded design; both the operating surgeons and patients were non-blinded. All surgeries were conducted by two oral and maxillofacial surgeons, each with over 10 years of clinical experience, who underwent standardized training before the study to ensure consistent surgical procedures. Postoperative assessments, including pain, swelling, mouth-opening limitation, and bone density evaluation, were performed by two independent oral radiologists who received calibration training and remained blinded to group assignments throughout the study. To ensure consistency in postoperative pain assessment, the evaluators also underwent standardized training and achieved an intra-group consistency coefficient (ICC> 0.85).
