研究目的：

建立京津冀区域社区自然人群队列3.5万人。动态观测京津冀地区重大慢性疾病的患病率及相关危险因素流行规律和趋势，综合分析环境、生活行为方式、遗传等多种暴露因素对重大慢性疾病发生发展的影响。结合京津冀地区慢病特点，开发个体-社区-健康系统的电子健康管理系统，建立个体化慢病全程管理系统；构建疾病风险预测模型，建立风险评估体系。采用暴露组学、生命组学和毒理基因组学等方法，筛选大气污染相关心肺疾病生物标志物。  

Objectives of Study：

To establish cohort based on 350,000 community population in Beijing-Tianjin-Hebei. Observe the prevalence of major chronic diseases and related risk factors in the Beijing-Tianjin-Hebei region and the regularty of epidemic trends dynamicly and analyse the environmental, lifestyle behavior, genetic and other exposure factors on the occurrence and development of major chronic diseases. To develop an individual-community-health system e-health management system based on the characteristics of chronic diseases in the Beijing-Tianjin-Hebei region. To construct disease risk prediction models and risk assessment system . Using methods such as exposure histology, biopsies, and toxicogenomics to screen biomarkers of air pollution-related cardiopulmonary diseases.

采用多阶段分层整群抽样方法。第一阶段，采用整群抽样，选取京津冀不同地理区域（西北部、中部、东南部）的5个城市。第二阶段，考虑到居民的依从性、人口的稳定性和当地的医疗条件，由当地疾病预防控制中心选择了各市的3-5个社区。最后阶段，在选定社区，签署知情同意书的，符合纳入标准的候选人纳入研究样本。

Randomization Procedure (please state who generates the random number sequence and by what method)：

The cohort recruited participants using multistage stratified cluster sampling method. In stage I, five cities of Beijing-Tianjin-Hebei region were selected by simple cluster sampling. In stage II, 3-5 communities were selected by local centers for disease control. In stage III, participants were recuit in the study.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

纸质问卷，调查相关文件等原始记录均保存于首都医科大学临床流行病学实验室内，录入资料及原始数据保存于项目组专用云盘。
