研究目的:

中心静脉穿刺技术是临床上常用的穿刺技术,传统的中心静脉穿刺技术是基于识别解剖结构以及触摸静脉旁的动脉搏动来实现的。这一传统的穿刺技术并不能应对解剖变异的患者,且在血栓形成高危患者行中心静脉导管放置的过程中出现血栓脱落等风险进而对患者造成严重后。但是,超声引导下的穿刺可以很容易地看到解剖结构和确认静脉通畅,有助于避免意外的动脉穿刺。此外,在特殊的临床情况下,基于动脉脉搏触诊的标志性技术具有挑战性或不可能(例如,心肺复苏期间中心静脉导管(CVC)的放置等)。 增强现实(AR)技术的发展促进了可视化的发展,佩戴AR设备链接超声机,超声的实时影像以及现实周围环境视野将同时清晰的呈现在佩戴者的眼前。本研究主要是探讨AR技术在超声引导下穿刺等操作上的应用优势,能更好的做到手眼协调,降低穿刺操作的复杂性,降低操作时间和并发症发生率、提高操作者的满意度。  

Objectives of Study:

The central venous puncture technique is a commonly used puncture technique in clinical practice. The traditional central venous puncture technique is based on identifying anatomical structures and touching the arterial pulsation next to the vein. This traditional puncture technique cannot deal with patients with anatomical variations, and there is a risk of thrombosis during the placement of central venous catheters in patients at high risk of thrombosis. However, ultrasound-guided puncture can easily see the anatomical structure and confirm the patency of the vein, which helps to avoid accidental arterial puncture. In addition, in special clinical situations, the landmark technique based on arterial palpation is challenging or impossible (for example, the placement of central venous catheter (CVC) during cardiopulmonary resuscitation). The development of augmented reality (AR) technology has promoted the development of visualization. Wearing an AR device linked to an ultrasound machine, the real-time image of ultrasound and the surrounding environment will be clearly displayed in front of the wearer. This study mainly discusses the application advantages of AR technology in ultrasound-guided puncture and other operations, which can better achieve hand-eye coordination, reduce the complexity of puncture operations, reduce the operation time and complication rate, and improve the satisfaction of operators.

排除标准:

1: 穿刺部位感染、外伤、烧伤或皮肤破损等 2: 颈内静脉血栓或可疑血栓 3: 严重凝血功能障碍(INR > 1.5,血小板 < 50×10⁹/L 或近期使用抗凝药物未停药) 4: 孕妇或哺乳期妇女

Exclusion criteria:

1: Infection at the puncture site, trauma, burns, or skin breakage, etc 2: Internal jugular vein thrombosis or suspicious thrombosis 3: Severe coagulopathy (INR > 1.5, platelets < 50×10⁹/L or recent use of anticoagulants that have not been discontinued) 4: Pregnant or lactating women
