研究目的：

本项目旨在探索新型、磁共振导航、非侵入性神经调控rTMS技术，对服用抗精神病药物治疗的精神分裂症超重受试者体进行精准干预，调控其食欲渴求和体重，评估其疗效、安全性，并探究其潜在的作用机制。  

Objectives of Study：

The aim of this project is to explore a novel, magnetic resonance navigation, non-invasive neuromodulation rTMS technology for precise intervention in overweight subjects with schizophrenia treated with antipsychotic drugs, regulating their appetite and weight, evaluating its efficacy and safety, and exploring its potential mechanisms of action.

药物成份或治疗方案详述：

（1）导航下rTMS技术疗效评估：设计随机对照试验，运用标准化测评工具包括体重指数（BMI）、阳性症状和阴性症状量表（PANSS）、视觉食物模拟量表（VAS）等，评估导航下rTMS调控精神分裂症超重（服用抗精神病药物治疗后体重超重及以上）受试者食欲渴求和体重减轻技术的疗效； （2）导航下rTMS技术对神经认知功能影响：利用神经心理状态测验（RBANS），分析导航下rTMS治疗前后精神分裂症超重受试者注意力、记忆力、执行功能等神经认知功能的变化，评估其改善效果。 （3）导航下rTMS技术安全性评估：监控导航下rTMS治疗精神分裂症超重受试者过程中可能出现的不良反应，记录和分析治疗相关的不良事件，确定其严重程度和发生率，评估rTMS技术的安全性。 （4）rTMS技术对结构和功能MRI的影响分析：rTMS治疗前在MRI导航下进行精确定位，采集结构和功能MRI数据，评估rTMS治疗精神分裂症超重受试者前后额叶前皮质及中脑皮层奖赏回路功能连接性的变化和影响，分析其可能神经机制作用。 （5）导航下rTMS技术作用机制探讨：分析导航下rTMS治疗对神经递质系统、神经可塑性、功能连接等方面的影响，探讨导航下rTMS技术精准干预精神分裂症超重受试者不同脑区对食欲渴求的神经调节和神经认知功能改善的潜在神经生物学机制。 （6）导航下rTMS技术长期疗效与生活质量评估：进行长期跟踪研究，通过标准化的精神病症状评分量表和生活质量评估工具，评估导航下rTMS技术的长期疗效和对精神分裂症受试者生活质量的改善。 

Description for medicine or protocol of treatment in detail：

(1) Efficacy evaluation of rTMS technology under navigation: Design a randomized controlled trial using standardized assessment tools including body mass index (BMI), positive and negative symptom scale (PANSS), visual food analog scale (VAS), etc., to evaluate the efficacy of rTMS in regulating appetite craving and weight loss in subjects with schizophrenia overweight (overweight or above after taking antipsychotic medication) under navigation; (2) The impact of navigation based rTMS technology on neurocognitive function: Using neuropsychological state tests (RBANS), analyze the changes in neurocognitive functions such as attention, memory, and executive function in overweight subjects with schizophrenia before and after navigation based rTMS treatment, and evaluate the improvement effect; (3) Safety assessment of rTMS technology under navigation: Monitor potential adverse reactions during the treatment of overweight subjects with schizophrenia using rTMS under navigation, record and analyze treatment-related adverse events, determine their severity and incidence, and evaluate the safety of rTMS technology; (4) Analysis of the impact of rTMS technology on structural and functional MRI: Accurate localization was performed under MRI navigation before rTMS treatment, and structural and functional MRI data were collected to evaluate the changes and effects of functional connectivity in the prefrontal cortex and midbrain cortex before and after rTMS treatment in overweight subjects with schizophrenia, and to analyze their possible neural mechanisms. (5) Exploration of the mechanism of action of rTMS technology under navigation: Analyze the effects of rTMS treatment under navigation on the neurotransmitter system, neuroplasticity, functional connectivity, and other aspects, and explore the potential neurobiological mechanisms of precise intervention of rTMS technology under navigation on the neural regulation of appetite craving and improvement of neurocognitive function in different brain regions of overweight subjects with schizophrenia. (6) Long term efficacy and quality of life assessment of rTMS technology under navigation: Conduct a long-term follow-up study to evaluate the long-term efficacy and improvement of quality of life in schizophrenia subjects using standardized psychiatric symptom scoring scales and quality of life assessment tools. 

纳入标准：

Inclusion criteria

排除标准：

①符合ICD-10诊断标准的其他重性精神障碍者。 ②有头部严重损伤史或癫痫发作史者。 ③目前使用人工耳蜗、起搏器、体内金属植入物和fMRI或rTMS的其他禁忌症者。 ④过去一年内使用减肥药物或极低热量饮食、怀孕或哺乳、饮食紊乱或物质依赖者。 ⑤有不稳定的心血管疾病者（最近1年内的心肌梗死或中风、心力衰竭、急性肢体缺血、严重的外周动脉闭塞性疾病）。 ⑥有其他肥胖的潜在原因者（甲状腺功能减退症、库欣综合征、性腺功能减退症、胰岛素瘤）。 ⑦近半年内使用过无抽搐电休克、tDCS等其他物理治疗者。 ⑧既往史提示或研究者判断目前或过去6个月内有明显自杀和暴力倾向受试者，或基于研究者临床评估认为存在自杀和暴力行为风险者，或者经研究者评估认为不适合参加试验者。 ⑨怀孕、哺乳或在试验期间计划妊娠者。

Exclusion criteria：

1.Individuals with other severe mental disorders that meet the diagnostic criteria of ICD-10 Individuals with a history of severe head injuries or epileptic seizures Currently, there are other contraindications for the use of cochlear implants, pacemakers, in vivo metal implants, and fMRI or rTMS. 2.Individuals with a history of severe head injuries or epileptic seizures. 3.Currently, there are other contraindications for the use of cochlear implants, pacemakers, in vivo metal implants, and fMRI or rTMS. 4.Individuals who have used weight loss drugs or extremely low calorie diets within the past year, are pregnant or breastfeeding, have suffered from eating disorders or substance dependence Individuals with unstable cardiovascular disease (myocardial infarction or stroke within the past year, heart failure, acute limb ischemia, severe peripheral arterial occlusive disease). 5.Individuals with unstable cardiovascular disease (myocardial infarction or stroke within the past year, heart failure, acute limb ischemia, severe peripheral arterial occlusive disease). 6.Individuals with other potential causes of obesity (hypothyroidism, Cushing's syndrome, hypogonadism, insulinoma). 7.Individuals who have used non convulsive electroconvulsive therapy, tDCS, and other physical therapies within the past six months. 8.Subjects with a history indicating or determined by the researcher to have a significant tendency towards suicide and violence in the current or past 6 months, or those at risk of suicide and violence based on the researcher's clinical assessment, or those deemed unsuitable to participate in the trial by the researcher's assessment. 9.Pregnant, breastfeeding, or planning to conceive during the trial period.
