研究目的：

建立一个大型的前瞻性无症状颅内动脉粥样硬化患者脑卒中一级预防队列，明确无症状颅内动脉狭窄患者的危险因素分布、狭窄动脉分布，确定无症状人群中利用 TCD 筛选颅内动脉狭窄的最佳前哨动脉；结合高分辨磁共振检查观察颅内动脉粥样硬化的斑块进展和回缩自然史；通过临床随访首次卒中事件和第二次卒中事件，建立颅内动脉粥样硬化性斑块易损性识别的评价与转归体系方法；结合血管危险因素和狭窄血管的血流动力学参数，建立无症状颅内动脉粥样硬化患者认知下降风险预警模型。  

Objectives of Study：

To establish a large prospective first-order prevention cohort of asymptomatic patients with intracranial atherosclerosis for stroke, determine the distribution of risk factors and stenotic arteries in asymptomatic patients with intracranial artery stenosis, and determine the best sentinel artery for screening intracranial artery stenosis by TCD among asymptomatic people; The natural history of plaque progression and retraction of intracranial atherosclerosis was observed with high-resolution magnetic resonance imaging; Through clinical follow-up of the first stroke event and the second stroke event, the evaluation and outcome system methods for identifying the vulnerability of intracranial atherosclerotic plaque were established; Combining vascular risk factors and hemodynamic parameters of stenotic vessels, an early warning model of cognitive decline risk in asymptomatic patients with intracranial atherosclerosis was established.

纳入标准：

Inclusion criteria

排除标准：

1.临床确诊的非动脉粥样硬化相关的颅内动脉狭窄； 2.已诊断为阿尔茨海默病、路易体痴呆、额颞叶变性、帕金森病等认知功能下降的神经系统变性疾病； 3.严重的神经功能缺陷，使患者无法独立生活，预期无法完成随访计划；或合并其他可能影响终点事件判断和随访的疾病，如严重的心、肺、肝、肾、内分泌疾病等； 4.颅外颈部血管狭窄＞50％； 5.有核磁成像的安全性有关的相对/绝对禁忌症（体内金属，幽闭恐惧症等）； 6.颞窗透声性差； 7.拒绝签署知情同意书。

Exclusion criteria：

1.Clinically confirmed non atherosclerosis related intracranial artery stenosis; 2.Diagnosed with neurodegenerative diseases such as Alzheimer's disease, Lewy body dementia, frontotemporal lobe degeneration, Parkinson's disease, and cognitive decline; 3.Severe neurological deficits prevent patients from living independently and are expected to be unable to complete follow-up plans; Or merge other diseases that may affect the judgment and follow-up of endpoint events, such as severe heart, lung, liver, kidney, endocrine diseases, etc; 4.Extracranial neck vascular stenosis>50%; 5.Relative/absolute contraindications related to the safety of magnetic resonance imaging (such as in vivo metal, claustrophobia, etc.); 6.Poor sound transmission through the temporal window; 7.Refusal to sign informed consent form.
