纳入标准：

Inclusion criteria

排除标准：

患者排除标准 (1) 患有严重精神障碍疾病者； (2) 有恶性肿瘤等合并其他严重疾病的患者； (3) 无照顾者陪同者。 照顾者排除标准 (1) 伴有严重慢性病且病情不稳定者； (2) 已参加过相关培训者。

Exclusion criteria：

Patient exclusion criteria (1) Those with severe mental disorder diseases; (2) Those with malignant tumors or other serious diseases in combination; (3) Those who are not accompanied by a caregiver. Exclusion Criteria for Caregivers (1) Those with serious chronic diseases and unstable conditions; (2) Those who have participated in relevant training.

常规护理和宣教：由责任护士向患者讲解与疾病相关的知识（如：降压药物的合理使用、饮水控制、体重管理等）、监测记录患者的生命体征、观察与护理患者的病情。
