研究目的:

1、通过前瞻性队列研究监测自然分娩产妇体温,测定产妇血浆中的炎性因子、检测胎盘信号通路的变化,明确胎盘信号通路能够参与并调节ERMF。 2、根据ERMF中胎盘合体滋养层细胞的不同反应,筛选能够导致胎盘出现这种现象的病理生理变化。 3、前瞻性队列研究检测产妇血浆中胎盘来源的细胞成分鉴别诊断感染和ERMF导致的产妇发热。 4、临床随机对照研究糖皮质激素针对性预防血浆里细胞外囊泡中IL-6/IL-10比值升高的产妇发生ERMF可行性。  

Objectives of Study:

1. Monitor the body temperature of natural laborers, measure inflammatory factors in maternal plasma, detect changes in placental signaling pathways, and clarify that placental signaling pathways can participate in and regulate ERMF through prospective cohort studies. 2. According to the different responses of placental syncytiotrophoblast cells in ERMF, the pathophysiological changes that can lead to this phenomenon in the placenta are screened. 3. Prospective cohort studies were conducted to detect placenta-derived cellular components in maternal plasma, and to differentially diagnose infection and maternal fever caused by ERMF. 4. Clinical randomized controlled study of the feasibility of glucocorticoids for the prevention of ERMF in pregnant women with an elevated IL-6/IL-10 ratio in extracellular vesicles in plasma.

药物成份或治疗方案详述:

1、前瞻性队列研究验证胎盘参与并调节ERMF 通过前瞻性队列研究监测自然分娩产妇的体温、收集产妇外周血和胎盘标本,验证产妇血浆中炎性因子变化与ERMF的关系、验证胎盘分泌的细胞因子参与调节ERMF、验证影响胎盘分泌细胞因子的信号通路。 2、检测产妇胎盘衰老、缺血缺氧、氧自由基损伤、激素分泌异常(孕激素、雌激素、催产素等)与ERMF的关系 通过检测自然分娩产妇胎盘中的衰老、缺血缺氧、氧自由基损伤、激素分泌异常(孕激素、雌激素、催产素等)指标,寻找能够改变胎盘JAK-STAT3-SOCS3-IL-10和JAK1-MAPK-IL-6信号通路的病理生理变化,阐明胎盘调节ERMF的机制。 通过滋养体细胞系和原代培养的滋养体细胞来验证不同的病理生理改变能否导致滋养细胞对炎性因子刺激生产不同的反应。 3、检测产妇血浆中胎盘来源的细胞成分鉴别诊断感染和硬膜外分娩镇痛导致的产妇发热。 本项目利用胎盘合体滋养体细胞和皮肤的表皮一样,不断地脱落,不断更新,甚至把多余的排出合体滋养体,这些脱落的细胞成分(囊泡),通过子宫静脉,到达心脏,多在肺部血管中被清除。由于这些细胞成分中含有大量的信号分子、炎性因子,使得通过检测分娩产妇外周静脉血,分析胎盘来源的细胞成分,判断胎盘的炎性反应成为可能。通过检测硬膜外分娩镇痛发热和感染发热产妇的外周滋养层细胞来源的囊泡,确定它们改变胎盘的差别,为鉴别诊断硬膜外分娩镇痛发热和感染发热提供依据。 

Description for medicine or protocol of treatment in detail:

1. Prospective cohort studies verified that the placenta participates in and regulates ERMF Through prospective cohort studies, the body temperature of spontaneous mothers in labor was monitored, peripheral blood and placental samples were collected, the relationship between the changes of inflammatory factors in maternal plasma and ERMF was verified, the cytokines secreted by the placenta were verified to participate in the regulation of ERMF, and the signaling pathways affecting the secretion of cytokines by the placenta were verified. 2. Detect the relationship between maternal placental aging, ischemia and hypoxia, oxygen free radical damage, abnormal hormone secretion (progesterone, estrogen, oxytocin, etc.) and ERMF By detecting aging, ischemia and hypoxia, oxygen free radical damage, and abnormal hormone secretion (progesterone, estrogen, oxytocin, etc.) in the placenta of natural laborers, the pathophysiological changes that can change the placental JAK-STAT3-SOCS3-IL-10 and JAK1-MAPK-IL-6 signaling pathways were investigated, and the mechanism of placental regulation of ERMF was clarified. Trophozoite cell lines and primary cultured trophoblasts were used to verify whether different pathophysiological alterations could lead to different trophoblast production responses to inflammatory factor stimuli. 3. Detection of placenta-derived cell components in maternal plasma, differential diagnosis of infection and maternal fever caused by epidural labor analgesia. This project uses placental syncytiotrophoblasts, like the epidermis of the skin, to constantly exfoliate, renew, and even excrete excess syncytiotrophozoites, and these shed cellular components (vesicles) pass through the uterine veins, reach the heart, and are mostly removed in the blood vessels of the lungs. Because these cellular components contain a large number of signaling molecules and inflammatory factors, it is possible to determine the inflammatory response of the placenta by detecting the peripheral venous blood of the laboring woman and analyzing the cell components derived from the placenta. By detecting the peripheral trophoblast-derived vesicles of epidural labor analgesia and infected febrile parturients, the differences between them in altering the placenta were determined, which provided a basis for the differential diagnosis of epidural labor analgesic fever and infectious fever.

乙肝患者、艾滋病患者、孕高症患者、胎儿窘迫产妇、前置胎盘患者等重症产妇。

Hepatitis B patients, AIDS patients, patients with high pregnancy, fetal distress mothers, placenta previa patients and other severely ill mothers.
