研究目的：

利用上海国际旅行卫生保健中心（上海海关口岸门诊部）现有的高通量测序平台和技术，我们旨在优化并标准化针对上海市第六人民医院神经外科重症患者中合并感染的各种类型样本的预处理程序和核酸提取技术。通过建立基于宏基因组下一代测序（mNGS）的感染病原体筛查技术，我们将能够比较mNGS与传统检测手段在神经外科重症监护室的检测效果，描绘神经外科常见合并感染的病原体谱系。对于确诊感染的患者，我们将利用基因组信息指导针对性用药，并根据病原体的丰度及时调整治疗方案。这种方法将基于宏基因组技术建立一个精准治疗计划，提高神经外科合并感染患者快速治疗感染的能力。此外，它还将扩大第二代测序仪器的应用范围，并提高其利用率。

Objectives of Study：

:Leveraging the existing high-throughput sequencing platform and technologies at the Shanghai International Travel Health Care Center (Shanghai Customs Port Outpatient Clinic), we aim to optimize and standardize the preprocessing procedures and nucleic acid extraction techniques specifically for various types of samples from neurosurgical intensive care patients with concurrent infections at the Sixth People's Hospital of Shanghai. By establishing an infection pathogen screening technology based on metagenomic next-generation sequencing (mNGS), we will be able to map the spectrum of common concurrent infections. For patients with confirmed infections, we will utilize genomic information to guide targeted medication and adjust the treatment in a timely manner according to the abundance of pathogens. This approach will establish a precision treatment plan based on metagenomic technology, enhancing the ability to rapidly treat infections in neurosurgical patients with concurrent infections. Furthermore, it will expand the application scope of second-generation sequencing instruments and improve their utilization rate

Inclusion criteria

排除标准：

1、患者诊断排除感染； 2、无法获取临床样本； 3、患者基线资料不完整，基线资料出现异常值； 4、样本质量不合格(未建库成功)、mNGS质量控制未通过； 5、患者拒绝给予知情同意。

Exclusion criteria：

1. Diagnosis and exclusion of infection; 2. Unable to obtain clinical samples; 3. The patient's baseline data is incomplete, and there are abnormal values in the baseline data; 4. The sample quality is unqualified (the library is not successfully established), and the mNGS quality control is not passed; 5. The patient refuses to give informed consent.

Target condition (The target condition is a particular disease or disease stage that the index test will be intended to identify. Please specify the characteristics in detail; the population should has a complete spectrum and good representative):	According to the "Chinese Expert Consensus on the Diagnosis and Treatment of Infections in Neurosurgical Critical Care Patients (2017)" and patients in the neurosurgical intensive care unit with clinical symptoms highly suggestive of infection, infections are categorized into: (1) Pulmonary infection group, including hospital-acquired pneumonia (HAP) and ventilator-associated pneumonia (VAP); (2) Central nervous system infection group; (3) Other types of infection group, such as bloodstream infections, urinary tract infections, gastrointestinal infections, etc.; (4) Non-infection group.
