研究目的：

（1）分析高原地区留置鼻胃肠管患者发生非计划拔管的影响因素。 （2）根据影响因素，制定预防高原地区成人非计划拔管的干预措施。  

Objectives of Study：

(1) Analysis of the influencing factors of unplanned extubation of patients with indwelling nasogastric and gastrointestinal ducts in the plateau area. (2) Based on influencing factors, formulate intervention measures to prevent unplanned extubation of adults in plateau areas.

纳入标准：

Inclusion criteria

排除标准：

①患有鼻咽部及口腔炎症、肿瘤、感觉异常等疾病者； ②已参加其他实验者，医嘱禁忌者； ③合并严重并发症而无法顺利完成调查者，如伤口严重感染、伤口造口漏、大出血等。

Exclusion criteria：

1.Persons suffering from nasopharyngeal and oral inflammation, tumors, and abnormal sensory diseases; 2.Persons who have participated in other experiments and are contraindicated by doctors; 3.Persons who have combined severe complications and cannot complete the investigation smoothly, such as severe wound infection and wound stoma leakage , heavy bleeding, etc.

常规护理方法： 1、置管前健康宣教：由置管护士讲解操作目的、配合方法及心理疏导；2、置管后采用“人字型”3M鼻贴固定于鼻尖部，再采用 “一字型”3M胶带在脸颊二次固定； 3、负压求固定：卧床患者统一使用曲别针固定在同侧枕边，下床活动的患者统一固定在同侧的肩部，引流液及时观察、倾倒及记录； 4、每天早晚两次口腔护理。

After pipe placement, use a "herring font" 3M nose patch to fix it on the tip of the nose, and then use a "one-line" 3M The tape is fixed on the cheeks again; 3.Negative pressure fixation: Patients in bed use paper pins to fix them on the ipsilateral pillow, and patients who are moving out of bed are fixed on the shoulders on the ipsilateral shoulders, and drainage fluid is observed, poured and recorded in time; 4.Daily Oral care twice in the morning and evening.

质量改善专案措施： 1、护理人员系统培训及考核：内容包括留置胃管操作流程、非计划拔管风险评估、管道的固定方式、非计划拔管应急预案、相关知识考核。 2、置管前石蜡油充分润滑管道。 3、置管后测量患者鼻部皮肤水分及油脂度含量，使用酒精棉片脱脂，再用吸油纸擦拭患者鼻部，充分吸干鼻部油分及水分。 4、管道固定：统一使用3M胶带固定，将传统的“人字型”固定法改为“大字型”固定，该方法除了鼻尖及胃管固定外，再在鼻翼两侧加固一条，在靠近鼻孔的胃管上黏贴一圈水胶体敷料，防止管道压迫鼻腔造成鼻腔黏膜的压疮，二次固定位置更换于患者耳垂部，固定时运用“高举平台法”，注意避免过度牵拉。 5、健康宣教：置管前、置管中、置管后实施医护一体化及家属全程参与的健康教育模式，采用视频、PPT、宣教小册子等方式讲解，并不定期针对患者的掌握情况进行反馈。 6、增加患者鼻咽部舒适度：石蜡油滴鼻，每天早上8：00及晚上20:00点，使用5ml注射器在置管侧鼻腔滴入1ml，滴注时注意避免接触水胶体敷料及鼻贴，防止鼻贴脱落造成非计划拔管，每天早9：00、中午14：00及下午19:00点给予0.9%氯化钠溶液含漱，漱完口以后给与布地奈德雾化吸入； 7、监测环境温湿度，当温度>24度，及时开窗通风，当温度<18度嘱患者多穿衣服注意保暖；环境湿度<50%时，给与加湿器，当环境湿度>60%时关闭加湿器，及时开窗通风，空气对流。 8、加强护理巡视：按照分级护理级别及时巡视，观察管道固定是否牢固，若有卷边、潮湿等及时更换；引流液超过1/3或者1/2后给予及时倾倒或更换； 9.严格交接班制度。

Paraffin oil fully lubricate the pipeline before placing the pipe. 3. After the tube is placed, measure the moisture and oil content of the patient's nose, use alcohol cotton pads to degreas, and then wipe the patient's nose with oil-absorbing paper to fully absorb the oil and moisture in the nose. 4.Pipe fixation: Use 3M tape to fix it uniformly, and change the traditional "herring font" fixation method to "large font" fixation. In addition to fixing the tip of the nose and gastric tube, this method strengthens one on both sides of the nose wing, and is close to the nostrils. A circle of hydrocolloid dressing is pasted on the gastric tube to prevent the pipe from compressing the nasal cavity and causing pressure ulcers in the nasal mucosa. The secondary fixing position is replaced on the patient's earlobe. When fixing, use the "height platform method" to avoid excessive pulling. 5.Health education: Implement medical and nursing integration and family participation in the whole process before, during and after pipe placement. It uses videos, PPTs, education booklets and other methods to explain, and provides feedback on the patient's understanding from time to time. 6.Increase the comfort of the patient's nasopharyngeal: paraffin oil drops the nose. Use a 5ml syringe to drip 1ml in the nasal cavity on the side of the cannula. Pay attention to avoid contact with hydrocolloid dressings and nasal patches during dripping. Prevent the nose patch from falling off and causing unplanned tube extubation. Gargle 0.9% sodium chloride solution every day at 9:00 am, 14:00 noon and 19:00 pm, and give budesonide atomization and inhalation after rinsing the mouth; monitor Ambient temperature and humidity.7.when the temperature is > 24 degrees, open the windows for ventilation in time, and ask the patient to wear more clothes and keep warm; when the ambient humidity is < 50%, give the humidifier, and turn off the humidifier when the ambient humidity is > 60%. , open the windows in time for ventilation and air convection. 8.Strengthen nursing inspections: inspect in a timely manner according to the graded nursing level, observe whether the pipeline is fixed firmly, and replace it in time if there is curled edges, dampness, etc.pour or replace it in time after the drainage liquid exceeds 1/3 or 1/2; 9.transfer the shift system.
