研究目的：

本研究拟以早期前房穿刺放液紧急降眼压治疗急性闭角型青光眼合并白内障为切入点，探索一种合理的新的治疗策略，以期能快速降低眼压、并早期手术治疗，缩短住院日、提高患者生活质量，寻找一种易推广的原发性急性闭角型青光眼合并白内障诊疗新策略。本研究拟在前期研究基础上，进行前瞻性随机对照研究，以期进一步获得原发性急性闭角型青光眼合并白内障诊疗新策略的高级别循证医学证据。

Objectives of Study：

This study intends to early paracentesis of anterior chamber reducing IOP in acute angle closure glaucoma with cataract as the breakthrough point, to explore a reasonable new treatment strategies, in order to reduce intraocular pressure, rapid and early surgical treatment, shorten the hospitalization time, improve the quality of life of patients, looking for a kind of primary acute angle closure the diagnosis and treatment of glaucoma with cataract easy to promote the new strategy. Based on previous studies, we conducted prospective randomized controlled trial in order to further obtain evidence-based medical evidence of new strategies for diagnosis and treatment of primary acute angle closure glaucoma with cataract.

纳入标准：

Inclusion criteria

排除标准：

a. 继发性青光眼
b. 既往眼部手术史
c．既往眼前节炎症性病变
d．除青光眼、轻中度白内障外尚有其他眼部疾病的患者
e. 角膜内皮细胞≤1000 个／mm2
f．角膜透明，可以立即进行激光虹膜周切术者
g. 对醋甲唑胺或甘露醇等治疗有医学禁忌症
h. 糖尿病患者
i．已知的对降眼压药物有高敏感反应病史者
j．已经过药物、前房穿刺、激光等干预治疗者
k．孕妇、独眼、残障人士
L. 不能签署知情同意者

Exclusion criteria：

1. secondary glaucoma;
2. history of eye surgery;
3. Anterior Of inflammatory disease;
4. patients with other ocular diseases in addition to glaucoma and mild and moderate cataracts;
5. of corneal endothelial cells is less than 1000 / mm2;
6. cornea is transparent and can be performed immediately by laser iris circumcision medical contraindications to vinegaramine or mannitol treatment;
7. patients with diabetes;
8. is known to have a history of highly sensitive response to intraocular pressure lowering drugs;
9. has been treated with drugs, anterior chamber puncture, laser and other intervention treatment;
10. pregnant, single eyed, disabled people.
