慢性失眠(Chronic insomnia disease,CID)是一种普遍存在的睡眠障碍,对个人的生活质量和健康有显著影响。既往研究表明,慢性失眠不仅影响睡眠质量,还对认知功能产生负面影响,包括注意力、记忆力和执行功能的损害。视空间工作记忆(Visuo-spatial working memory,VSWM)是指处理和暂存视觉和空间信息的能力,在日常生活和工作中发挥着关键作用。例如,导航、阅读地图和组织视觉信息等任务都依赖于这种认知功能。VSWM作为一种核心的认知功能,受慢性失眠的影响尤为显著。慢性失眠可能通过多种机制影响VSWM功能,包括改变神经元活动、神经递质水平和脑电生理特征。这些变化可能集中在大脑的特定区域,如前额叶皮层(PFC),这一区域被认为与工作记忆和执行功能密切相关。功能近红外光谱(fNIRS)是一种先进的神经成像技术,通过量化穿过大脑皮质的近红外光被血红蛋白吸收的情况,实时观察和监测脑皮质血流动力学的变化,提供关于任务执行过程中脑功能变化的动态视角。与以往用于失眠研究的影像学工具相比,fNIRS具有高时间分辨率、非侵入性、成本较低和操作便捷等优点。近年来,fNIRS技术在认知神经科学研究中得到了广泛应用,尤其在研究动态认知任务和小样本研究中具有优势。 本研究旨在利用fNIRS技术深入探讨慢性失眠患者在VSWM任务中的脑功能变化。通过对比CID患者与健康对照组(HC)在VSWM任务中的脑活动差异,详细分析CID对特定脑区的影响,我们期望能够揭示CID在VSWM任务中大脑功能的复杂作用机制。这一研究将为临床实践提供新的诊断和治疗思路。通过揭示慢性失眠患者的脑功能特点,有望为个性化的认知干预措施提供理论依据,提高治疗效果和患者生活质量。  

Objectives of Study:

Chronic Insomnia Disease (CID) is a prevalent sleep disorder that significantly impacts individuals' quality of life and health. Previous studies have shown that chronic insomnia not only affects sleep quality but also negatively impacts cognitive functions, including attention, memory, and executive function. Visuo-spatial working memory (VSWM), which refers to the ability to process and temporarily store visual and spatial information, plays a crucial role in daily life and work. Tasks such as navigation, map reading, and organizing visual information rely heavily on this cognitive function. As a core cognitive function, VSWM is particularly affected by chronic insomnia. CID may impair VSWM through various mechanisms, including alterations in neuronal activity, neurotransmitter levels, and electrophysiological characteristics. These changes may be concentrated in specific brain regions, such as the prefrontal cortex (PFC), which is closely associated with working memory and executive function. Functional near-infrared spectroscopy (fNIRS) is an advanced neuroimaging technique that quantifies the absorption of near-infrared light by hemoglobin as it passes through the cerebral cortex, enabling real-time observation and monitoring of cortical hemodynamic changes. This provides a dynamic perspective on brain function during task execution. Compared to traditional imaging tools used in insomnia research, fNIRS offers advantages such as high temporal resolution, non-invasiveness, cost-effectiveness, and ease of operation. In recent years, fNIRS has been widely applied in cognitive neuroscience research, particularly in studies involving dynamic cognitive tasks and small sample sizes. This study aims to use fNIRS technology to explore the brain functional changes in CID patients during VSWM tasks. By comparing the differences in brain activity between CID patients and healthy controls (HC) during VSWM tasks, we will analyze the impact of CID on specific brain regions in detail. We hope to uncover the complex mechanisms underlying brain function in CID patients during VSWM tasks. This research will provide new insights for clinical diagnosis and treatment. By revealing the brain functional characteristics of CID patients, it is expected to offer a theoretical basis for personalized cognitive interventions, thereby improving treatment outcomes and patients' quality of life.

纳入标准:

Inclusion criteria

排除标准:

(1)意识障碍史;(2)既往有其他类型的睡眠障碍,睡眠呼吸暂停、不宁腿综合征、周期性肢体睡眠中的运动,或昼夜节律睡眠障碍(例如,早睡期综合征)等;(3)目前使用的已知会导致失眠的药物(如类固醇);(4)其他病史:如中枢神经系统感染、头颅损伤、癫痫、多发性硬化、中毒代谢性疾病、帕金森病、颅内肿瘤、糖尿病等躯体疾病等;有重度抑郁症、双相障碍、严重焦虑症、精神分裂症、人格改变等精神疾病史;(5)有酒精、药物、咖啡依赖史;(6)存在磁共振检查禁忌症。

Exclusion criteria:

(1) History of disturbance of consciousness; (2) a history of other types of sleep disorders, such as sleep apnea, restless leg syndrome, movement during periodic limb sleep, or circadian rhythm sleep disorders (e.g., early sleep syndrome); (3) Current use of drugs known to cause insomnia (such as steroids); (4) Other medical history: such as central nervous system infection, head injury, epilepsy, multiple sclerosis, toxic metabolic diseases, Parkinson's disease, intracranial tumors, diabetes and other physical diseases; History of major depression, bipolar disorder, severe anxiety disorder, schizophrenia, personality change and other mental illness; (5) A history of alcohol, drug, coffee dependence; (6) There are contraindications for MRI.
