研究目的：

圆锥角膜是以角膜中央变薄向前突出，呈圆锥形为特征的一种眼病。它常造成高度不规则近视散光和不同程度的视力损害，不伴有炎症。本病多发生于20岁左右的青年，通常为双眼先后发病，原因不明。眼镜和隐形眼镜可用于圆锥角膜早期,而疾病晚期时可采用部分前板层角膜移植术（anterior lamellar keratoplasty，ALK）、前部深板层角膜移植术或穿透性角膜移植术（penetrating keratoplasty，PK）等。此外,角膜胶原交联术可以阻止圆锥角膜进展，角膜胶原交联术（corneal collagen cross-linking,CXL）是一种针对原发或继发性圆锥角膜等角膜疾病的新疗法，可以分为去上皮角膜交联，跨上皮交联，准分子激光治疗性角膜切削术（PTK）联合CXL、准分子激光屈光性角膜切削术（PRK）联合CXL等方法。其中PTK联合CXL可分为普通PTK或优化的PTK联合CXL，其中普通的PTK是以全角膜直径正中心为中点进行切削，优化的PTK是根据角膜地形图结果以角膜突出最顶点（即最薄处）为中点进行切削，切削直径比普通PTK更小，本研究旨在评估优化后的PTK在圆锥角膜治疗的临床疗效，以及其对控制圆锥角膜病程的长期作用。

Objectives of Study：

Keratoconus is an eye disease characterized by thinning and protrusion of the central cornea in a conical shape. It often causes high irregular myopic astigmatism and varying degrees of visual impairment without inflammation. This disease mostly occurs in young people around the age of 20, usually affecting both eyes successively, with an unknown cause. Glasses and contact lenses can be used in the early stage of keratoconus, while in the advanced stage, anterior lamellar keratoplasty (ALK), deep anterior lamellar keratoplasty or penetrating keratoplasty (PK) can be adopted. In addition, corneal collagen cross-linking (CXL) can prevent the progression of keratoconus. CXL is a new therapy for primary or secondary keratoconus and other corneal diseases, which can be divided into epithelium-off CXL, transepithelial CXL, phototherapeutic keratectomy (PTK) combined with CXL, photorefractive keratectomy (PRK) combined with CXL, etc. Among them, PTK combined with CXL can be further classified into conventional PTK or optimized PTK combined with CXL. Conventional PTK is performed with the center of the entire corneal diameter as the midpoint for ablation, while optimized PTK is based on the results of corneal topography, with the most protruding point of the cornea (i.e., the thinnest point) as the midpoint for ablation, and the ablation diameter is smaller than that of conventional PTK. This study aims to evaluate the clinical efficacy of optimized PTK in the treatment of keratoconus and its long-term effect on controlling the course of keratoconus.

排除标准：

怀孕、哺乳或计划怀孕的女性；任一眼出现任何眼前段或眼后段的急性感染性或非感染性眼部疾病的患者；既往的眼内或角膜手术史；角膜基质厚度过薄的患者；瘢痕体质、糖尿病、胶原性疾病等可能影响角膜伤口愈合；系统性红斑狼疮和类风湿性关节炎，手术后易引起角膜溶解；任何研究者认为不适合参加该临床试验的情况。研究方法

Exclusion criteria：

Females who are pregnant, breastfeeding, or planning to become pregnant; Patients with acute infectious or non-infectious ocular disease of any anterior or posterior segment of the eye in either eye; History of previous intraocular or corneal surgery; Patients with thin corneal stromal thickness; Scar constitution, diabetes, collagen diseases, etc. may affect corneal wound healing; Systemic lupus erythematosus and rheumatoid arthritis, which can easily cause corneal lysis after surgery; Any condition that, in the opinion of the investigator, is not suitable to participate in this clinical trial.
