研究目的：

本研究旨在验证医院为主导的院外心脏康复健康管理模式的效果，经随机对照研究设计，对该模式进行实证研究，为今后开展院外心脏康复提供参考。  

Objectives of Study：

This research aims to verify the effectiveness of the hospital-led health management model for post-hospital cardiac rehabilitation. Through a randomized controlled study design, an empirical research on this model is conducted to provide a reference for future implementation of post-hospital cardiac rehabilitation.

纳入标准：

Inclusion criteria

排除标准：

1.未控制的高血压（安静时收缩压≥160mmHg 或舒张压≥100mmHg）； 2.合并心、肝、肾功能不全。

Exclusion criteria：

1. Uncontrolled hypertension (systolic blood pressure >= 160 mmHg or diastolic blood pressure >= 100 mmHg at rest); 2. Comorbid with heart, liver or kidney dysfunction.

According to the inclusion and exclusion criteria, the subjects were determined on the basis of informed consent by researcher Wu Lin in the inpatient wards of the department of cardiology of two hospitals.
