研究目的:

1. 比较试验组与对照组义齿清洁方法在减少义齿上机会性呼吸道病原体方面的效果(通过呼吸道病原体的检出率、丰度及微生物组指数[ORPI]评估)。 2. 比较试验组与对照组义齿清洁方法在提高清洁程度方面的效果(通过半自动平面图像分析法评估菌斑面积百分比)。 3. 比较试验组与对照组义齿清洁方法在减少义齿性口腔炎方面的效果(依据Newton分级评估)。 4. 比较试验组与对照组义齿清洁方法在患者满意度方面的差异。  

Objectives of Study:

1. To compare the efficacy of the test and control denture cleaning interventions in reducing opportunistic respiratory pathogens on dentures [as assessed by the prevalence, abundance, and microbiome-based index of respiratory pathogens (ORPI)]. 2. To compare the efficacy of the test and control denture cleaning interventions in improving denture cleanliness (as assessed by the percentage plaque area coverage using semi-automated planimetric assessment). 3. To compare the efficacy of the test and control denture cleaning interventions in reducing denture stomatitis (as assessed by Newton’s Classification). 4. To compare the efficacy of the test and control denture cleaning interventions in terms of patient satisfaction.

Inclusion criteria

排除标准:

1. 受试者使用留置鼻胃管或经皮内镜胃造口,无法正常进食饮水。 2. 吸烟者(包括电子烟和嚼烟)。 3. 被诊断患有获得性免疫缺陷综合征、控制不良的糖尿病、贫血、口干症或免疫抑制状态的受试者。 4. 在招募前4周内或随访期间使用类固醇治疗、抗生素/抗真菌药物,或接受放射治疗/化疗的受试者。 5. 使用义齿黏着剂重衬、修复或固位的义齿。 6. 义齿部分或完全不适合佩戴,或存在断裂情况。 7. 仅在用餐时佩戴义齿的受试者。

Exclusion criteria:

1. Participants have an indwelling nasogastric feeding tube or percutaneous endoscopic gastrostomy (unable to eat and drink normally). 2. Smoker (including e-cigarettes and tobacco chewing). 3. Participants are diagnosed with acquired immune deficiency syndrome, uncontrolled diabetes mellitus, anaemia, xerostomia, or immunosuppression. 4. Use of steroid treatment, antibiotics/antifungal medication, or undergoing radio- or chemotherapy within the past 4 weeks before recruitment and during follow-up. 5. The denture has been relined, repaired, or retained using denture adhesive products. 6. The denture with partially or completely ill-fitting/fracture. 7. Participants who are wearing dentures during meals only.

干预组受试者将每日使用便携轻型的超声波清洁器 “Lohas ultrasonic cleaner GXZ UC01”(Lohas Technology (Int.) Ltd., HK)和酶性过氧化物类义齿清洁片 “Polident 3 min?”(GlaxoSmithKline Healthcare, Moon Township, PA)进行义齿清洁。 具体方法如下: 在超声波清洁槽中加入225毫升自来水,加入并溶解一片 “Polident 3 min?” 清洁片。将义齿浸入该溶液中,并使用 “Lohas ultrasonic cleaner GXZ UC01” 以45kHz频率进行15分钟的超声波清洁。清洁后用流动水冲洗10秒并妥善存放。

Intervention:

In the intervention arm participants will receive a portable and lightweight ultrasonic cleaner (Lohas ultrasonic cleaner GXZ UC01, Lohas Technology (Int.) Ltd., HK) AND enzymatic peroxide-based denture cleanser tablets (Polident 3 minTM, GlaxoSmithKline Healthcare, Moon Township, PA) to use daily. The ultrasonic bath will be filled with 225 ml of tap water to which the enzymatic peroxide-based denture cleanser tablet will be added and dissolved. The denture will be immersed in the ultrasonic bath and denture cleanser solution and then subjected to ultrasonic cleaning at 45kHz frequency for 15 minutes. The denture will be rinsed with running water for 10 seconds and stored.

Intervention:

To standardise the conventional mechanical brushing approach all participants will receive a commercially available denture brush (GUM denture toothbrush, Sunstar Singapore Pte. Ltd.) AND enzymatic peroxide-based denture cleanser tablets (Polident 3 minTM, GlaxoSmithKline Healthcare, Moon Township, PA) to use daily. A non-ultrasonic bath will be filled with 225ml tap water to which the enzymatic peroxide-based denture cleanser tablet will be added and dissolved. The denture will be immersed in the non-ultrasonic bath and denture cleanser solution for 15 minutes, followed by standardised brushing using the enzymatic peroxide-based denture cleanser solution for 30 seconds. The denture will be rinsed with running water for 10 seconds and stored.

Participants will be block randomised with a block of four (ABBA) into ARM 1 or ARM 2 of the study. The sequence of randomisation will be created by a computer random digit generation system conducted by a technical officer of the Translational Research Laboratory in the Faculty of Dentistry not involved in data collection or analyses.
